[Objective] By observing the changes of the concentration of serum lipoprotein associated phospholipids A2 (Lp-PLA2), and the correlation between serum lipoprotein associated phospholipid A2 (Lp-PLA2) and vascular endothelial growth factor (VEGF) in patients with diabetic retinopathy at different stages. To explore the role of cytokines in the occurrence and development of diabetic retinopathy, and the prospect of be used as the pathogenesis risk evaluation index of diabetic retinopathy and the treatment effect assessment indicators for clinical application.[Methods] All inductees was February 2014 to September 2015 Kunming General Hospital of Chengdu Military Region treated type 2 diabetes patients, and all the patients were after mydriasis fundus photography or fundus fluorescein angiography. According to the standards of international clinical staging in 2002,51 cases of non-diabetic retinopathy group (group NDR),67 cases of Non-proliferative diabetic retinopathy group (NPDR group) and 42 cases of proliferative diabetic retinopathy group (PDR group) were included in the study. Measure fasting venous blood triglyceride (TG), total cholesterol (TC), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), lipoprotein associated phospholipase A2 (Lp-PLA2)and vascular endothelial growth factor (VEGF) of all the selected objects. Comparative analysis of measurement data. All data were analyzed by SPSS software. Measurement data between the two groups were compared using t test, count data were compared with x2 test, correlation analysis using Pearson linear correlation analysis, Spearman linear correlation analysis and multiple linear regression, P<0.05 was considered statistically significant.[Results] Comparison of biochemical indexes among the groups: â‘ The levels of Lp-PLA2 and VEGF in fasting venous blood of NPDR group were significantly higher than those in NDR group, the difference was statistically significant (P<0.05). â‘¡ The levels of Lp-PLA2 and VEGF of fasting venous blood in PDR group were significantly higher than those in NPDR group, the difference was statistically significant(P<0.05). â‘¢The levels of TG, LDL-C and VEGF were significantly correlated with the concentration of Lp-PLA2 in all the selected objects(P<0.05).[Conclusions] â‘  The levels of serum lipoprotein-associated phospholipase A2 and vascular endothelial growth factor were closely related to the occurrence and progression of diabetic retinopathy. â‘¡ The level of serum lipoprotein-associated phospholipase A2 in patients with type 2 diabetes mellitus has a predictive value for the risk of diabetic retinopathy and the severity of diabetic retinopathy. â‘¢The development of diabetic retinopathy may be prevented or delayed by decreasing serum lipoprotein associated phospholipase A2 levels and regulating lipids.[Objectives] By observing the effect of fenofibrate treatment on changes in levels of lipoprotein-associated phospholipase A2, Vascular endothelial growth factor and the visual function, retinal morphology in diabetic retinopathy patients, to explore the application value of fenofibrate in prevention and treatment of diabetic retinopathy.[Methods] A total of 67 patients (134 eyes) with non-proliferative diabetic retinopathy (NPDR group) were selected from Kunming General Hospital of Chengdu military region from February 2014 to September 2015, and were randomly divided into two subgroups:the experimental group and the control group. Meanwhile,42 cases (84eyes) with proliferative diabetic retinopathy (PDR group) were randomly divided into two subgroups:the experimental group and the control group. The cases of control group were treated with routine drug treatment, the cases of experimental group were given routine drugs plus fenofibrate treatment for 12 weeks. To observed and compared the vision and the inspection result of fundus photography before and after treatment in two groups of patients, and the changes in levels of serum Lp-PLA2. All the experimental data entry SPSS 17.0 statistical software for processing, Take t test method to test the measurement data in the test, if P<0.05, the difference was considered statistically significant.[Results] â‘  NPDR group:After 12 weeks of drug intervention, compared with before treatment, the levels of Lp-PLA2 and VEGF in experimental group of patients were decreased, by statistical analysis, the difference was statistically significant (P<0.05). â‘¡ PDR group:After 12 weeks of drug intervention, compared with before treatment, the levels of Lp-PLA2 and VEGF in experimental group of patients were decreased, the difference was statistically significant (P< 0.05). â‘¢ NPDR group:After treatment, compared with the control group, the effective number of eyes of which vision and fundus were improved in the experimental group was more than that in the control group, and the effective rate of vision and fundus treatment in the experimental group was was significantly higher than that of the control group, and the difference was statistically significant (x2=4.352, P<0.05).[Conclusions] â‘  Fenofibrate can significantly reduce the levels of serum Lp-PLA2 and VEGF in patients of diabetic retinopathy. â‘¡ Fenofibrate treatment of clinical non-proliferating diabetic retinopathy, can significantly improve the visual acuity, can effectively delay the progression of retinopathy, protect the eyes.
