Part 1 Study of factors affecting awareness of non-alcoholic fatty liver disease among the publicObjective:To investigate the awareness status of NAFLD and related factors among the public so as to guide the implementation of awareness efforts,achieving more optimized disease prevention and management.Methods:Our research group have screened from the 850 respondents(patients and whose relatives and friends) which come from 5 municipal level hospitals in Kunming were investigated with the self-designed questionnaire,which involved the knowledge related to NAFLD and affecting factors.The questionnaire in addition to consisted of general information,such as age, gender, nationality,place of residence, level of education,occupation, etc. but also includes related history of fatty liver, related basic knowledge and risk factors and preventive measures of NAFLD().All of the respondents answer the same questionnaire.Using descriptive statistics to analyze the awareness of public. Using SPSS 20.0 to build the multi-factor unconditioned Logistic regression model and evaluate its effectiveness by receiver operating characteristic(ROC) curve.Results:1. The awareness of the general population,patients with fatty liver and the NAFLD high-risk group is 60.5%,70.16%,62.6% respectively.Among them,patients with fatty liver have the highest NAFLD awareness not only in the aspect of general awareness,but also in the aspect of basic NAFLD knowledge,risk factors and preventive measures,However there has no distinction between the general-population and the NAFLD high-risk groups in any aspect awareness of NAFLD.Its a pitty that patients with fatty liver which are the most reluctant to accept fatty liver related health education.Finally,whether the awareness in general,basic NAFLD knowledge or preventive measures,patients with fatty liver have better awareness than the group which have not yet been diagnosed with fatty liver.2.7 items were found to be related to the awareness among public.Of those gender(OR 1.560;95%CI 1.067 to 2.281),level of education is College or above(OR 2.778;95%CI 1.083 to 7.125),occupation is neither farmer nor unemployed(OR 2.259; 95%CI 1.345 to 3.793),brain work(OR 2.259;95%CI 1.345 to 3.793),whether to accept medical health mission(OR 4.139;95%CI 2.531 to 6.769),willing to accept health mission(OR 4.993;95%CI 2.065 to 12.073),past medical history with fatty liver disease(OR 1.881;95%CI 1.051 to 3.365)were associated with increased NAFLD awareness among public. However, nationality, age, marital status, BMI, occupational stress, income levels, living environmental, associated with the metabolic syndrome had nothing to do with the NAFLD awareness among public.3.The accuracy of Logistic regression model evaluated by ROC curve was 79.40%.4.The public have higher interest in preventive measures related NAFLD than other aspect NAFLD knowledge. The group which have higher awareness of NAFLD, which have higher interest in the knowledge related NAFLD.Conclusions:1. NAFLD awareness of public is extremely scarce, especially in the aspect of basic knowledge is very low.Even though the public have better awareness and higher interest in the aspect of preventive measures,but rarely real active implementation in which.Health education and popularization of fatty liver related knowledge in the public should be enhanced to prevent the the the prevalence of the disease.2. Female,level of education is College or above,occupation is neither farmer nor unemployed,brain work,had accepted medical health mission,willing to accept health mission,past medical history with fatty liver disease were protective factors of awareness among public related NAFLD.Our study haven’t found nationality, age, marital status,BMI,occupational stress, income levels,living environmental,associated with the metabolic syndrome had anything to do with the NAFLD awareness among public.3. Willing to accept health mission or had accepted medical health mission,past medical history with fatty liver disease were the protective factors of NAFLD awareness among public,which further demonstrated the significance of medical health mission.However,there are only 26.6% of the respondents had accepted medical health mission in our study,which suggested that the medical health mission in our country is weak extremely.4. Of all factors,brain wor.k and level of education is College or above,had accepted medical health mission,which which once again stressed the necessity and significance to improve the universal literacy,which is the security of carrying out NAFLD health education smoothly.Part 2 Study of factors affecting awareness of non-alcoholic fatty liver disease among the providersObjective:To investigate the awareness status of NAFLD and related factors among the providers so as to set up standardization management mechanism of NAFLD and the prevention of NAFLD in further.Methods:Our research group have screened from the 424 on-the-job medical workers which come from gastroenterology dept, neurology, endocrinology, diabetes, heart and liver disease of 13 different level hospitals in Kunming were investigated with the self-designed questionnaires,the questionnaires involved general information,the knowledge related to NAFLD and affecting factors.The questionnaire in addition to consisted of general information,such as age, gender, level of education,the hospital grade,the department,health care class title,length of service, etc. but also includes related basic knowledge and diagnostic, screening, treatment and follow-up, health education about NAFLD.All of the respondents have given the same questionnaire.Using descriptive statistics to analyze the awareness of public. Using SPSS 20.0 to build the multi-factor unconditioned Logistic regression model and evaluate its effectiveness by receiver operating characteristic(ROC) curve.Results:1.The general cognition of NAFLD among providers was 53.58±10.30,pass rate was 24.8%. Among of which,the cognition of related basic knowledge of NAFLD was 56.06±13.38,pass rate was 40.03%,the cognition of diagnostic and screening knowledge about NAFLD was 53.68±15.01,pass rate was 37.0%,the cognition of treatment about NAFLD was 45.94±14.98,pass rate was 17.5%.The cognition of treatment about NAFLD is scarce extremely among providers.2.Nearly half of the providers (40.6%) know nothing about NAFLD guidelines, most of the medical staff screen and evaluate the curative effect of NAFLD were according to the serum transaminase.But in fact, the vast majority (82.5%) of the medical staff dont understand the serum transaminase characteristics of patients with NAFLD. Almost 67.0% of the providers have never delivered any health education of NAFLD to patients,but most of the medical personnel (87.1%) expressed a willingness to accept further NAFLD knowledge education and training.3.3 items were found to be related to the awareness among providers.Of those the hospital grade (OR 2.507;95%CI 1.106 to 5.684),the department (OR 2.540;95%CI 1.497 to 4.311), Willing to accept the fatty liver knowledge education and training (OR 4.975;95%CI 2.196 to 11.271) were associated with increased NAFLD awareness among the providers.However, gender,age,level of education,health care class title, length of service had nothing to do with the NAFLD awareness among medical stuffs. 4.The accuracy of Logistic regression model evaluated by ROC curve was 74.1%.Conclusions:1.The cognition of NAFLD was very low among providers,especially in the aspect of treatment about NAFLD.2.The hospital grade,the department, willing to accept the fatty liver knowledge education and training were associated with increased NAFLD awareness among providers.However, gender,age,level of education,health care class title,length of service had nothing to do with the NAFLD awareness among medical stuffs.
