| Objective The experiment aimed to detect the correlation between the change of plasma glutamate〠gamma aminobutyric acid and S100β protein content and early cognitive dysfunction in infant after propofol or etomidate anesthesia laparoscopic hernia repairment.Methods One hundred infants of 1-3 years old, scheduled for elective laparoscopic hernia neoplasty, randomly divided into propofol anesthesia group(group P) and etomidate anesthesia group(group E), 50 cases in each group; 50 cases of healthy infants of 1-3 years old selected as control group(group C).Group P, E were conducted anesthesia induction of propofol 1-3 mg kg-1,etomidate 0.1-0.4 mg kg-1, respectively. P,E group were assessed the cognitive function according to Bailey infant cognition scale Ⅲ(BSID-Ⅲ) at 1 d before operation, 3 d after operation and 3 months after operation, group C was assessed according to the Z score of postoperative cognitive dysfunction at the same time. Group P and E were separately divided into postoperative cognitive dysfunction group and non postoperative cognitive dysfunction group After surgery according to the occurrence of postoperative cognitive dysfunction.At the same time, group P and E were measured plasma glutamate, gamma aminobutyric acid and S100β protein content at the time of pre-operation and before goig out from the anesthesia recovery room; Compared the plasma glutamate with gamma aminobutyric acid content in group P and E,also between group P and E after operation in the cognitive dysfunction group andnon post operative cognitive dysfunction group.Results Cases of postoperative cognitive dysfunction in group P and E were 10(20%) and 9(18%, P>0.05), respectively. Compared with the pre-operation, there was a slight increase in plasma glutamate, gamma- amino acid and S100β protein content in group P and E in non postoperative cognitive dysfunction group, showed no statistically significant difference(P>0.05),but there was a significant increase in postoperative cognitive dysfunction group(P<0.05); plasma glutamate, gamma aminobutyric acid and S100β protein content in Group P and E were incresed when assessed before going out the from the anesthesia recovery room, showed a positive correlaton with the occurence of postoperative cognitive dysfunction 3 days after operation.Conclusion Plasma glutamate, gamma aminobutyric acid and S100βprotein content can be a indirectly refection of the degree of brain injury of infants after anesthesia, could be a detection index for early cognitive dysfunction in infants after operation. |
