Objective:To discuss the effect and significancce of the oxidative stress and inflammatory reaction in patients with ACI,by investigating the content changes of serum oxidized low density lipoprotein(ox-LDL),high-sensitivity C-reactive protein(hs-CRP) and serum lipids in patients with acute cerebral infarction(ACI) and their clinical significance.Methods:We selected a total of 60 patients with initial acute cerebral infarction within72 hours who treated in the Department of Neurology,First Affiliated Hospital of Soochow University from March 2014 to June 2014 as case group.All patients, diagnosis were up to the diagnostic criteria revised by the fourth national cerebrovascular disease conference.At the same time,another 30 heathy population chosen from Physical Examination Department in this hospital were as the contral group.According to the gender,disease severty(NIHSSgrades),carotid arterial plague and the prognosis(mRSscores),we divided these patients into different groups.The serum ox-LDL,hs-CRP and serum lipids(total cholesterol 、 triglyceride 、 low density lipoprotein-cholestero 、 high density lipoprotein-cholesterol) were detected by enzyme-linked immune sorbent assay(ELISA),latex-enhanced immunonephelometic method and enzymatic colorimetry.All the dates were analyzed by SPSS18.0.Results:1.The content of the serum ox-LDL,hs-CRP and lipids had no statistical difference between men and women(P<0.05).2.The conent of the serum lipids in patients with ACI:The content of the serum TC,LDL-C in patients with ACI was higher than which in the control group,but the content of the serum TG,HDL-C had no statistical difference between the the two groups;The content of the serum lipids showed no statistical difference in comparison among the light,medium and severe group,as well between the good and poor prognosis group;The content of the serum lipids showed no statistical difference in comparison among the unstable,stable and no plaque group except the comarison between the unstable and no plaque group;There was no linear correlation between the serum lipids level and NIHSS grades,mRS scores and the level of the serum ox-LDL.3.The conent of the serum ox-LDL in patients with ACI:The content of the serum ox-LDL in patients with ACI was obviously higher than which in the control group;The content of the serum ox-LDL in patients with ACI was highest in the severe group,the medium group taken second,the light group came last,and the difference was statistically significant;The serum ox-LDL level in patients with ACI was highest in the unstable plaque group,the stable plaque group taken second,the no plaque group came last,and the difference was statistically significant;The serum ox-LDL level of the patients with ACI in the poor prognosis group was higher than which in the good prognosis group;The content of the serum ox-LDL had a positive relationship with the NIHSS grades and mRS scores.4.The conent of the serum hs-CRP in patients with ACI:The content of the serum hs-CRP in patients with ACI was obviously higher than which in the control group;The content of the serum hs-CRP in patients with ACI was highest in the severe group,the medium group taken second,the light group came last,and the difference was statistically significant;The content of the serum hs-CRP in other the unstable or stable plaque group was higher than which in the no plaque group,but there was no statistical difference between the two groups;The serum hs-CRP level of the patients with ACI in the poor prognosis group was higher than which in the good prognosis group;The content of the serum hs-CRP had a positive relationship with the NIHSS grades,mRS scores and the content of the serum ox-LDL.5.The positive rate of the combined detection for the serum ox-LDL and hs-CRP was higher than any of its separate detection in pations with ACI.Conclusions:1.Compared with the serum lipids,the content of serum ox-LDL and hs-CRP is more related to the disease severity,prognosis and carotid plaque in the patients with ACI,prompting that there is a high level of the oxidative stress and inflammatory reaction in the patients with ACI.2.The united detecting of serum ox-LDL and hs-CRP can be more helpful to clinical forecast in the patients with ACI.3.Decreasing the level of serum hs-CRP and treatment by the way of anti-oxidation in the outbreak may improve the prognosis of the patients with ischemic stroke.
