Abstract/Summary:
Objective:In order to explore the effect of targeted drugs and traditional Chinese medicine in the field of modern medicine on lung cancer, we collected cases which treated by Erlotinib combined with Xiaoji Decotion. Then we used retrospective analysis and follow-up of clinical cases with observational studies for having some insight into the clinical efficacy and safety of integrated traditional Chinese and Western medicine. We tried to provide evidence of evidence-based medicine for the treatment of Non-Small Cell Lung Cancer. So that more patients can benefit from the reasonable treatment.Methods:In this study we used an observational single arm clinical study method and refered to some external references. A total of 186 patients with advanced Non-Small Cell Lung Cancer patients were collected according to the set of criteria. We reviewed and followed up the cases. The relevant clinical features of these cases included patients’gender, age, smoking status, medical disease, pleural effusion, pathological type, clinical stage, tumor metastasis, priority of targeted treatment, consumption of drinking Xiaoji Decotion, the CIK cell immunotherapy, PS score changes, tumor cell gene status, the length of time taking Erlotinib, the symptoms before and after the treatment and survival time, etc. Using SPSS statistical software, we built an observation database and conduct data analysis. Factors influencing efficacy and prognosis were analyzed.We made a horizontal comparison of our study and other trials in the international. Then we can describe the characteristics of the treatment in our study.Results:1.The 186 cases of patients with Non-Small Cell Lung Cancer who had received combined therapy showed us the median progression free survival was 9.988 months and the median overall survival was 20.9 months. Evaluation of curative effect in the near future, Erlotinib combined with traditional Chinese medicine in the treatment of around product evaluation of disease control rate was 91.4%, and the objective response rate was 37.6%. Whole case samples’half year survival rate was 86%, one year survival rate was 72%, two year survival rate was 29%. Adverse events during the treatment is mainly rash, diarrhea, liver damage. Other obvious rare serious adverse events were not observed. The rash total incidence rate was 32.3%, the total incidence of diarrhea was 12.9%, liver injury incidence was 7%. The efficacy of traditional Chinese medicine in the assessment of the total effective rate of up to 96.2%. TCM therapy compared with other treatment methods such as chemotherapy can make patients feel more obvious clinical manifestations of change.2. The effects of many clinical data on PFS were analyzed. Tumor gene status as an independent factor can affect the length of PFS. The gene status had guiding significance to the clinical choice treatment plan.3. According to the inclusion and exclusion criteria of several international classical clinical trials, we screened patients who meet the criteria from our database for the subgroup analysis. Compared to international study, we found that combined treatment had advantage in prolonging PFS and OS as second-line treatment. As the sametime, combined treatment had advantages in relieving the side effects as first-or second-line treatment.Conclusion:1. In the clinical observation of Xiaoji Decotion combined with Erlotinib in the treatment of NSCLC, EGFR gene mutation is an independent influencing factor in the progression free survival of patients with disease progression. The 19 and 21 exon mutation factors can prolong progression free survival. For improving the efficacy of therapy we should detect the genetic status of patient before treatment.2. Traditional Chinese medicine Xiaoji Decotion combined with targeted therapy Erlotinib can reduce the adverse reactions’incidence rate of anti-tumor therapy in first-line treatment and patients perceived discomfort, so as to improve patient compliance with treatment process.3. Traditional Chinese medicine Xiaoji Decotion combined with targeted therapy Erlotinib can play a certain role in increasing the efficiency of prolonging PFS and OS as second-line treatment.4. Oral preparation Xiaoji Decotion is highly practical. We found that combined therapy can reduce the incidence and severity of adverse reactions and improve the physical condition of the patients. Overall benefit is satisfactory.So Xiaoji Decotion can be used as a safe and effective adjuvant therapy in the treatment of cancer.
