BackgrandAging of population is a global public health and social problems.At present,Chinese aging of population is particularly serious,For the large elderly population,elderly population,high proportion of the old are not rich.The high prevalence of chronic diseases in the elderly and it is prone to a high incidence of falls injury.The cost due to a fall in hospital is enormous.On the one hand, patients would suffer from the pain of torture,the reduction of the quality of life, bear a heavy psychological burden;on the other hand,the family and society also carry a heavy economic burden.Therefore,it is an important research task and topic to analyze the hospitalization expenses and its composition,to explore the influencing factors and reduce the economic burden.ObjectiveIn this study,Analysis characteristics of the Hospitalized elderly fallen patients,hospitalization expenses and its composition influencing factors of a two level general hospital,s orthopedics department in Jinan,Put forward ountermeasures and suggestions,To provide a reference for reducing the burden of hospitalization expenses in elderly fallen patients. Data sources and resear methodsIn this study,Through the hospital information system, based on the information of the home page of the medical record,Collection 350 eligible cases from June 1,2013 to September 30,2015 of a two level general hospital,s orthopedics department in Jinan.Investigate Social and economic characteristics disease status,hospitalization expenses and its composition of the patients.SPSS20.0 software was used for descriptive analysis,the nonparametric rank sum test was used for univariate analysis,and multivariate linear stepwise regression method was used for multivariate analysis.To explore the impact of various factors on hospitalization expenses and its composition. Results1.In the Department of orthopedics,the elderly fallen hospitalized female patients,with fractures arein the majority.Patients with concomitant disease accounted for 88.57%.The injured part of the hip and lower extremity is the most common.The total hospitalization expenses were positively skewed distribution.Total hospitalization expenses,consumables costs accounted for the highest proportion of 53.04%,followed by drugs(的社会人口学特征、疾病情况等)and diagnostic(11.18%),the proportion of the treatment costs and comprehensive service fee,on behalf of the labor value of the medical staff is low.2.Single factor analysis:there were no statistical difference in the average hospitalization expenses and its composition of different sex,home address,marital status and work status;Only drugs cost have statistical difference in the age group;There was statistical difference in addition to the cost of treatment in fracture group;There was statistical difference in addition to the cost of blood products in payment method group;There was statistical difference in the cost of diagnosis among grouping of the number of concomitant diseases;There was statistical difference between the average hospitalization expenses and the cost of each branch in the groups of operation and hospitalization days.3.Multivariate analysis results:for the cos t of dependent variable,to take the e as the bottom of the logarithm,To age,work status,racture or no,surgery or no, the number of concomitant diseases,different payment methods,Hospitalization days as the independent variable,Multiple linear regression analysis was carried out to establish the regression equation:The average hospitalization expenses were related to the operation and hospitalization days,R2=0.572;The consumable costs were related to surgery,hospitalization,fracture,and the number of concomitant diseases, R2=0.876;The drug costs were related to surgery,hospitalization days and work status, R2=0.400;Treatment costs associated with surgery and hospitalization days,R2=0.750;Diagnosis costs were related to surgery,hospitalization days,the number of concomitant diseases and fracture,R2=0.326;Comprehensive service fees were related to surgery,hospitalization days,the number of concomitant diseases and medical payment methods,R2=0.722. Conclusions and suggests1.To focus on the elderly women,focus on the prevention of falls,to strengthen nursing care of patients with falls fracture,concomitant disease prevention and treatment.2.The features in the composition of hospitalization expenses are the proportion of the consumable costs, drugs and diagnostic costs, which is higher than that of comprehensive service fees and treatment costs,who reflecting the value of health care services.3.Hospitalization expenses in fallen patients is affected by many aspects,only comprehensive,fully taking into account the impact of each factor,we wound develop effective measures,and control the excessive growth of hospital costs.
