| The research background:Hypothyroidism during pregnancy is one of common diseases in endocrine diseases and correlates with many adverse pregnancy outcomes. In the early weeks of pregnancy, thyroid function of the baby has not been fully established. Thyroid hormone needed by the developing brain mainly comes from the mother. Once the mother can’t create enough thyroid hormone synthesis, it can affect fetal nerve mental development, and lead to increased perinatal period complications. It has become an important problem facing the clinicians that to correctly evaluate the thyroid hormone levels in pregnant women, especially during the important stage of the early weeks of pregnancy.If use the normal reference value range of pregnancy to evaluate the gestational thyroid hormone level, it can lead to pregnancy misdiagnosis and missed diagnosis. Pregnant women should use different gestational specific reference range screening for thyroid disease. Thyroid function reference is importmant to determine whether pregnant women in early pregnancy should be diagnosis as hypothyroidism during pregnancy and whether it should be treated.Objective:This study intends to detect the serum thyroid hormone (TT4), total three iodine thyroid original glycine (TT3), thyroid stimulating hormone (TSH), three free iodine thyroid original thyroid hormones (FT3) and free amino acid (FT4), thyroid peroxidase antibodies (TPOAb) from women in early pregnancy of Linyi city, in order to establish thyroid function reference range in Linyi early pregnant women, determine the linyi hypothyroidism diagnosis point in early pregnancy women, thus improve gestational thyroid disease prevention.Methods:The maternity clinic, and routine inspection of linyi han healthy pregnancy women in early pregnancy were selected from June 2014 to January 2015 in Linyi people’s hospital as experimental group, control group subjected from health physical examination center of Linyi people’s hospital of Linyi health of pregnant women. Epidemiological analysis software application of Epi info2002 sample size calculation, it was concluded that should investigate the linyi women in early pregnancy of 457 people, the control group of 120 people. All selected objects according to the survey will fill in the questionnaire, physical check-up, and qualified after extraction of fasting venous blood, using solid-phase chemiluminescence enzyme immunoassay method (ICMA) (Roche reagent) to detect serum index:serum thyroid hormone (TT4), total three iodine thyroid original glycine (TT3), thyroid stimulating hormone (TSH), three free iodine thyroid original thyroid hormones (FT3) and free amino acid (FT4), thyroid peroxidase antibodies (TPOAb).Test results used double double entry method entry Epidata 3.1 database, and used SAS 9.1 software for data analysis.Results:The serum FT3 level of Linyi women in early pregnancy is lower than it in the control group, the reference range is 3.77-6.03 pmol/L. Serum TSH level of Linyi women in early pregnancy is significantly lower than it in the control group (P<0.05), the reference range is 0.02-3.07 pmol/L. Serum FT4, TT3 and TT4 levels are higher than those in the control group (P<0.05), the reference range of serum FT4, TT3 and TT4 are 11.48-22.03 pmol/L,75.91-234.48 nmol/L and 5.67-18.94 nmol/L, respectively.Conclusion:The reference range of thyroid hormone of Linyi women in early pregnancy are TT375.91~234.48 nmol/L, TT45.67~18.94 nmol/L, FT3 3.77~6.03pmol/L, FT4 11.48~22.03 pmol/L, TSH 0.02~3.07 mIU/L.Linyi city female hypothyroidism during early pregnancy diagnosis point of contact for:TSH 3.07 mIU/L.
