| Objective: To study the predictive value of H-FABP in patients with sepsis.,and improve the survival rate of patients with severe sepsis. Methods: There was a prospective case-control study, which included 50 cases from October 2014- October2015.According to the international guidelines for severe sepsis and septic shock(2012),all cases were divided into sepsis group(16 cases),severe sepsis group(14 cases),septic shock group(20 cases); according whether the patients were survival within 28 days or not,all cases were divided to survival group(22 cases), non-survival group(28 cases). gender,age, nation, Acute Physiology and Chronic Health Evaluation score(APACHE II) was recorded, and H-FABP, B-type natriuretic, Creatine kinase, Creatine kinase isoenzymes,Troponin-T was tested and other indicators within 6 hours after admission. Doing t test,2? test, and the receiver operating characteristic curve by using SPSS21.0 statistical software. Results:H-FABP in septic shock group was higher than severe sepsis group and sepsis group(P<0.01). The 28-day mortality of septic group(80%) and severe sepsis group(71.4%) was higher than sepsis group(12.5%)(P<0.01).Non-survival’s H-FABP was higher than survivals(P<0.05). Non-survival’sBNP was higher than survivals(P<0.05).Non-survival’s cTn-T was higher than survivals(P<0.05).Non-survival’s CK was higher than survivals(P>0.05).Non-survival’s CK-MB was higher than survivals(P>0.05).Through the ROC of H-FABP(AUC:0.748,P:0.003,95%CI:0.605-0.890) and BNP(AUC:0.714,P:0.010, 95%CI:0.573-0.856),it shows that H-FABP is better than BNP.when H-FABP take 9.902ng/ml, the sensitivity of 82.1%, specificity of 63.6%.H-FABP,BNP has a certain value in 28-day mortality prediction. Conclusions: Sepsis shock has significantly higher 28-day mortality than sepsis and severe sepsis. Compared with BNP,CK,CK-MB,H-FABP has greater predictive value in sepsis patients, with exacerbations and incteased; also can predict 28-day mortality. |
