Endoscopic Surgery In Patients With Middle Ear Cholesteatoma

Objective:To explore the characteristics and clinical efficacy of endoscopic surgery by comparing with microscopic surgical method.To summarize the advantages and disadvantages of endoscopic surgery and some related factors and the treatment of postoperative complications,that aims to improve the surgical skills. Methods:The 29 cases clinical data of endoscopic surgery in patients with middle ear cholesteatoma and 47 cases by microsurgery in the The First Affiliated Hospital of Xinjiang Medical University from March 2014 to November 2015 were analyzed retrospectively.All the patients were under general anesthesia and all the pituitary adenomas were confirmed by cholesteatoma.All the clinical manifestation,imaging manifestations,the situation of operations,the consumed time of operations. treatment of postoperative complications,the postoperative length of staying in hospital and the postoperative follow-up situation were recorded and analysed. The two groups of clinical data were compared from the above aspects. Results:1.29 cases of endoscopic surgery in patients with middle ear cholesteatoma were successfully completed. Among the 27 patients followed up after 6months without recurrence(93.1%); 2 cases of residual after operation(6.9%). 1 cases of patients with postoperative appearance of delayed facial paralysis(3.45%), 3 cases preoperative CT showed bone of the skull base destroyer, 1 patients in cerebrospinal fluid leakage(3.45%); 1 cases paralysis patients before operation, postoperative symptom free significantly improved. The mean postoperative hospital stay are 2.51 days,the shortest of1 days,the longest 6 days. 2.In 47 patients with microscopic surgical method were successfully completed. Among the 43 cases, 6 patients were followed up for months without recurrence(91.49%), and 4 patients were found to have residual(8.01%). 11 cases of preoperative CT showed skull base destroyer, 2 patients appeared cerebrospinal fluidotorrhea(4.26%),3.cases patients with postoperative appeared delayed facial paralysis(6.38%). 2 cases of preoperative complete paralysis patients,postoperativefacial paralysis has no obvious improvement.The mean postoperativehospital stay are 5.6 days,the shortest of 4 days,the longest 8 days. Conclusion:comparing with microscopic surgical method, Endoscopic surgery has the folloing characteristics,more simple operation under direct vision,clearer and broader operative field,shorter postoperative hospital stay.It is a minimally invasive and safe surgical method.
