Objective: To explore the efficacy and safety of endoscpic surgery and microsurgery in the removal of the growth hormone pituitary adenomas.. Methods: Clinical data of 690 patients with pituitary adenomas were analyzed retrospectively. The single nostril transsphenoidal endoscopic surgery was performed on 379 patients(endoscopic group) and the single nostril transsphenoidal microsurgery on 418(microscope group). The safety and efficacy of two surgical methods were compared. Results: There were no significant differences in resection degree of tumors, surgical time and recurrence rate 2 years after surgery between the two groups(P>0.05). visual function recoveryrate, endocrine reief rate, average operative time(time from the use of epinephrine to shrink the nasal mucosa of nasal packing foam expansion),the average bleeding volume.Compared with microscope group, the total recovery of growth hormone was better, the incidence of complications was lower and hospital stay was shorter in endoscopic group(P<0.05). Conclusion: Transsphenoidal surgery of pituitary adenoma can not only make use ofendoscopic bu also under the microscope; Two surgical techniques inpostoperative hospitalization time were significantly different,Endoscopic surgery is superior to operation under the microscope. In bleeding volume, the effective rate of resection, postoperative complications, symptoms; and other aspects of tumorrecurrence in both groups no significant differences;the effective rate of resection of the two operative group Implied endoscopic surgery issuperior to prompt surgical microscope.
