Objectives: 1. To analyze the sero-conversion rate and risk factors among HIV discordant couples in Yili, Xinjiang.2. To explore the sero-conversion rate trend of HIV discordant couples who received antiretroviral therapy in Yili, Xinjiang. 3. To give reasonable suggestions for the work of prevention and control on local HIV discordant couples. Methods: Used ambispective cohort study. Set up the dynamic cohort of HIV discordant couples from January 1,2009 to December 31,2015, in Yili. Information included demography characteristics, biology information, behavioral information and the status of antiretroviral therapy. Negative spouse HIV antibody were screening every six months. Log rank and Cox regression model was implied to analyze the factors associated with sero-conversion. Results: 1. Among 1574 discordant couples, 49 HIV sero-conversion spouses were identified,and the sero-conversion rate was 1.1 per 100 person-years.2. The results from Cox analysis showed that age≤30, CD4+T lymphocyte counts≤350 cells/mm3,viral load≥400copy/ml, STD patients, inconsistent using of condom and untreated were risk factors. 3. Early antiretroviral therapy reduced the sero-conversion rate among HIV discordant couples effectively. Conclusion: In recent years, the sero-conversion rate stabilized among HIV discordant couples in Yili. In the future, the work of follow-up management should strengthen the screening of sexually transmitted disease and treatment, enlarge antiretroviral therapy coverage, monitor of CD4+T lymphocyte counts levels and viral load.
