Influence Of Qianlietongqiao Capsule On The Expression Of Bcl-2 And Bax Of The Detrusor Of Bladder In Rats With BPH And Clinical Trails

Objective:To explore the effectiveness of Qianlietongqiao Capsule in treating benign prostatic hyperplasia(BPH)(deficiency of the kidney and stasis)through clinical trails,and a further experimental research on influence of Qianlietongqiao capsule on the expression of Bcl-2 and Bax of the detrusor of bladder in rats with BPH aims at revealing the possible mechanism of Qianlietongqiao treating BPH.Methods:1.Clinical trials:90 patients who are eligible are randomly divided into treatment group( Qianlietongqiao capsule group) and control group( Longbishu capsule group),both 45.The treatment group receives Qianlietongqiao capsule and the control group receives Longbishu capsule.Then the overall curative effect and the change of pre- and post-medication international prostate symptom score(IPSS)、quality of life(Qo L)、maximum flow rate(Qmax)、residual urine volume、prostate volume and traditional Chinese medicine symptom scores(TCMSS)are observed and compared after 8 weeks.2.Experimental research:140 SD rats are needed,then 20 rats are randomly selected into sham operation group.BPH rat models are produced via castration and subcutaneous injection of testosterone propionate.Then the 120 successful established rat models are randomly divided as model group, Tamsulosin group, Longbishu capsule group, low-dosage Qianlietongqiao capsule group, moderate-dosage Qianlietongqiao capsule group and high-dosage Qianlietongqiao capsule group with20 rats in each group.Rats in model group are given 0.9%NaCl solution by gavage,and other groups are given corresponding drug suspension by gavage.After 30 days,all rats are executed and their detrusors are acquired to be tested.The expression of Bcl-2 mRNA and Bax mRNA in detrusor of bladder is detected by RT-PCR,and the ratio of Bax mRNA and Bcl-2 mRNA is also calculated.Results:1.Clinical trials:After treatment,the total effectiveness rate of treatment group and control group was 77.78% and 62.22% respectively, and there exists significant difference between the two groups(P<0.01).I-PSS,QoL,Qmax,residual urine volume and TCMSS are obviously improved in both groups after medication(P<0.01)except prostate gland volume(P>0.05);Compared with control group,treatment group makes better improvement of I-PSS, Qo L, Qmax and TCMSS(P<0.01),but decrease of residual urine volume has no obvious difference between the two groups(P>0.05).2.Experimental research:Compared with model group,difference of the expression of Bax mRNA and Bcl-2 mRNA and Bax/Bcl-2 in other groups is significant(P<0.05);Compared with Tamsulosin group,difference of the expression of Bax mRNA and Bcl-2 mRNA and Bax/Bcl-2 in low-dosage Qianlietongqiao capsule group is not obvious(P>0.05);Compared with Tamsulosin group,difference of the expression of Bax mRNA and Bcl-2 mRNA and Bax/Bcl-2 in moderate-dosage Qianlietongqiao capsule group and high-dosage Qianlietongqiao capsule group is significant(P<0.05 or P<0.01);Compared with Longbishu capsule group,difference of the expression of Bax mRNA and Bcl-2 mRNA and Bax/Bcl-2 in all Qianlietongqiao capsule groups is significant(P<0.05 or P<0.01).Conclusion:1.Qianlietongqiao capsule can significantly improve IPSS、Qo L、Qmax、residual urine volume and TCMSS of BPH patients(deficiency of the kidney and stasis),and it deserves a clinical promotion.2.Qianlietongqiao capsule can respectively increase and decrease the expression of apoptosis-regulatory genes,Bax mRNA and Bcl-2 mRNA.Simultaneously,the ratio of Bax mRNA and Bcl-2 mRNA rises.Qianlietongqiao capsule can accelerate apoptosisof detrusor.As the above mentioned,a part of mechanism that Qianlietongqiao capsule can treat BPH effectively is revealed.
