Oxcarbazepine And Valproate Monotherapy In Benign Childhood Epilepsy With Centrotemporal Spikes: A Attention Networks Improvement Evaluation

Objective To investigate the attention networks evaluation in children with benign epilepsy with centrotemporal spikes(BECT) treated by oxcarbazepine(OXC) or valproate(VPA) therapy.Methods The BECT children which 46 cases taken OXC and 31 cases taken VPA were fellowed up for 6 months and marked the clinical onset;All BECT children were given video-EEG test at baseline and after 6 months medicine treatment.Attention networks assessment was performed at baseline and after 6 months treatment with the efficiency of three anatomicaly defined attention networks.Results 1. During the follow-up, sustained cessation of seizures under medication was observed in 76.09%(35/46)of patients treated with OXC, clinical efficacy rate was 89.13%(41/46),It was higher in the OXC group compared with the VPA group 67.74%(21/31)and 83.87%(26/31)。But,there was no significantly difference between the two groups((X2=0.650,P>0.05),(X2=0.453,P>0.05). 2. 71.74%(33/46)showed an improvement in the grade of electroencephalographic pathology in OXC group,It was lower than VPA group 74.19%(23/31).But,there was still no significantly difference between the two groups(X2=0.056,P>0.05). 3. Intra-group comparison :The alerting、orienting and executive control networks effects of OXC group was 45(17,84)ms、18(-22,56)ms、59(19,106)ms at baseline.After 6 months of OXC monotherapy,three network effects was 73(40,99)ms、32(-6,71)ms、68(40,103)ms.The alerting and orienting networks effects havesignificant difference(P<0.05).The alerting 、 orienting、 and executive control networks effects of VPA group was 49(-22,102)ms、21(-18,45)ms、45(27,114)ms at baseline, After 6 months of VPA monotherapy,three network effects was 46(2,87)ms、17(-21,43)ms、58(8,80)ms,Three networks effects of VPA group after treatment was decreased,but there was no significant difference(P>0.05). 4. Comparison among groups:The alerting networks effects of OXC group was higher than VPA group,it had significant difference(P<0.05).The orienting、and executive control networks effects of two groups had no significant difference(P>0.05).The alerting networks effects 、the correct rate and the grand mean effect of OXC group were higher than that in VPA group,It had significant difference.Conclusion The BECT children treat with OXC or VPA has same effect on clinnical control and EEG improvement.OXC can preserve alerting 、orienting attention network functions as short-term monotherapy in children with typical BECTS.But,VPA can not improve the attention networks.The impact of OXC in aspect of alerting attention network functions get the advantage of VPA.
