Relative Factors Analysis On Sarcopenia In T2DM Patients

Objective:Sarcopenia is characterised as low mass, low muscle strength and low physical performance.Its incidence is rising with the growing elderly population in recent years,and it seriously affect the patients’ quality of life.It is associated with a variety of factors.This study is to explore sarcopenia’s relationship with gender,age,25-hydroxyvitamin D[25(OH)D] and the diabetic duration in type 2 diabetes mellitus(T2DM) patients.We hope that it can provide some clinical data to the prevention and treatment of sarcopenia.Methods:We enrolled 132 hospitalized patients with T2DM (41.6-88.4 years old) into the study,who were consistent with the diagnostic criteria for diabetes mellitus proposed by World Health Organization in 1999.Among the patients,67 were male,average age (62.10±10.55) years old,and 65 were female,average age (62.24±11.59) years old.We randomly chose 60 healthy individuals (40.9-88.3 years old) who accepted health examination during the same period as the contrast.26 were male among them,average age (63.96±13.76) years old,and 34 were female,average age (63.13±11.76) years old.All the objects were undergone dual energy X-ray absorption(DXA) to measure body composition,and the serum 25(OH)D level were measured by electrochemiluminescence.Sarcopenia was diagnosed by the Baumgartner diagnostic criteria.We compared the prevalence of sarcopenia in T2DM group and the control group,and in different gender respectively.The objects in the two groups were respectively divided into 1-5 groups according to age:group 1 (40.0-49.9 years old)、 group 2 (50.0-59.9 years old)、group 3 (60.0-69.9 years old)、group 4(70.0-79.9 years old)、group 5 (more than 80.0 years old).And they were respectively divided into vitamin D severe deficiency group(<10ng/ml),deficiency group(≥10-< 20ng/ml),insufficiency group(≥20-<30ng/ml) and sufficiency group(≥30ng/ml) according to the level of 25(OH)D.T2DM group was divided into a-e groups according to the diabetic duration:group a (<5.0 years)、group b(5.0-9.9 years)、group c (10.0-14.9 years)、groupd (15.0-19.9years)、groupe(more than 20.0years).We compared sarcopenia’s prevalence in each group.We used Logistic regression analysis to learn more about the risk and protective factors of sarcopenia.Results:(1) Compared with the control group (11.7%),the prevalence of sarcopenia in T2DM group (24.2%) was higher.(2) It was higher in men than in women (T2DM group:32.8%/15.4%; the control group:23.1%/2.9%, P<0.05).(3) The prevalence in more than 80-year-old objects (T2DM group:50.0%; the control group:42.9%) was the highest,and the objects with sarcopenia was older than those without sarcopenia (P<0.05).(4) The prevalence in vitamin D severe deficiency group (T2DM group:41.7%; the control group:30.0%) was the highest,and the 25(OH)D level of objects with sarcopenia was lower than those without sarcopenia (P<0.05).(5) The prevalence in more than 20 years diabetic duration group (61.1%) was the highest,and the diabetic duration of objects with sarcopenia was longer than objects without sarcopenia (P<0.05).(6) Logistic regression analysis showed that B value (Logistic regression coefficient) was greater than 0,and Exp(B) or OR was greater than 1 in gender,age and diabetic duration items of T2DM group.It was the same in gender and age items of the control group.B value was less than 0,and Exp(B) was less than 1 in 25(OH)D item of both groups.Wald test showed P<0.05.Conclusion:(1) Gender,age,T2DM and the diabetic duration are risk factors of sarcopenia.The incidence is more higher in male than in female.The higher the age, the higher the incidence of sarcopenia,and the longer the diabetic duration,the higher the incidence of sarcopenia.(2) Low 25(OH)D level is possibly associated with the occurrence of sarcopenia. Therefore, patients with vitamin D insufficiency or deficiency,especially the elderly male patients with T2DM,should supplement vitamin D to increase its level in order to reduce the possibility of the occurrence of sarcopenia.
