Object:(1) In order to explore the immune role that IL-17 may play in human cytomegalovirus disease, we detected the level of IL-17 in peripheral blood of infants diagnosed with cytomegalovirus disease.(2) To investigate the clinical characteristics of cytomegalovirus disease in infant under 3 months. (3) To investigate the clinical features of cytomegalovirus induced infectious mononucleosis-like syndrome in children.Methods:(1) We took infant’s veinal blood on empty stomach, testing the quantity of IL-17 by Cytometric Bead Array (CBA), then analysis the difference between two groups.(2) Retrospectively reviewed the information of 158 children diagnosed with cytomegalovirs disease in our hospital between 2010 January and 2014 December, including their laboratory examination results and clinical data.(3) Retrospectively reviewed the information of 81 children diagnosed with cytomegalovirs induced infectious mononucleosis-like syndrome in our hospital between 2010 January and 2014 December, including their laboratory examination results and clinical data. Compared with 30 EB virus induced infectious mononucleosis children.Results:(1) The difference of the IL-17 in peripheral blood between children with cytomegalovirus disease and healthy children was statistically significant. All of children in experimental group are diagnoesd with liver function damage, the alanine aminotransferase levels was positively correlated with the level of IL-17 in peripheral blood (r=0.926, P<0.001). (2) Among 158 children diagonesd with cytomegalovirus disease,17 cases were congenital cytomegalovirus disease (A group),141 cases were perinatal cytomegalovirus disease (group B). Children in two groups were all manifested as cough, tachypnea, three depression sign, convulsion, fever, hepatomegaly, jaundice, etc.Children with cough in group B reached 50.35%, and 23 of them with wheezing, accounting for 23.95%(23/96) of the pulmonary symptoms. In group A, there are 2 case of liver function damage accompanying hepatomegaly,53 case of liver function damage in group B,13 of them with hepatomegaly.42 of 158 cases involving multiple organ damage (26.58%):6 cases in group A, accounting for 35.29%, most of them are damaged in nervous system accompanying other system damage; 36 cases (25.53%) in group B, most of them are suffering from lung disease accompanying other system damage. Two children died in A group,accounting for 11.76%.One case died in B group, accounting for 0.7%. (3) 81 children with mononucleosis-like syndrome caused by CMV (CMV group) were aged from 1 to 11 years. The main clinical manifestations of the patients were fever, lymph node enlargement, angina, splenomegaly, palpebral edema, wheezing,etc. Compared with 30 EBV induced infectious mononucleosis children (EBV group), the thermal process of CMV group was (9.15±4.65) days, the difference between two groups was not statistically significant (t=0.979, P>0.05). The hospitalisation of patient in CMV group was (15.88 ±4.31) days, the difference between two groups was statistically significant (t=2.492, P<0.05). The WBC count of CMV group was (16.85±6.33)×109/L, the neutral granulocyte count of CMV group was (4.19±2.84)×109/L, and the lymphocyte count was (12.50±6.03)×109/L, the difference between two groups were not statistically significant. The platelet count of CMV group was (260.46±80.69) X 109/L, the difference between two groups was statistically significant (t=7.02,P<0.001).The abnormal lymphocytes count of CMV group was (0.27±0.22)×l09/L,the difference between two groups was statistically significant (t=3.814, P<0.001). The increase of abnormal lymphocyte has relationship with the hospitalisation.Concolusion:(1)After the infection of cytomegalovirus, the secretion of cytokines was imbalance, the virus can induce inflammation by increase the level of IL-17; The liver function damage of cytomegalovirus hepatitis is closely related to the increased secretion of IL-17, so we concluded that IL-17 may be involved in pathogenesis of cytomegalovirus hepatitis.(2) Cytomegalovirus disease in infant prone to suffer from multiple organ damage, especially congenital cytomegalovirus disease. Currently, congenital cytomegalovirus disease with multiple organ damage present as nonspecific symptoms and the prognosis is poorer.(3) Fever is the most common manifestations of cytomegalovirus induced infectious mononucleosis-like syndrome. The liver function damage of infectious mononucleosis-like syndrome caused by cytomegalovirus is better than EBV induced infectious mononucleosis, and the degree of abnormal lymphocytes increased is lower and recovered faster.
