Objective The objective of this study is to detect the immunoexpression of Extracellular Matrix Metalloproteinase Inducer(CD147, EMMPRIN) and Cyclophilin A(Cyp A) in periapical granulomas(PGs) and radicular cysts(RCs). The interaction of CD147 with Cyp A and their roles in periapical pathogenesis were also speculated.Methods Thirty PGs and Thirty-five RCs were collected after periapical surgery or extraction as the experimental samples from December 2014 to May 2015. Eight alveolar bone samples were obtained from alveoloplasty or extraction of impacted third molar as the control samples. Lesion size were recorded by CBCT. The protein expression of CD147 and Cyp A was measured by immunohistochemical analysis in all samples. Statistical analysis was performed to evaluate the differences of expression of CD147 and Cyp A according to the type of lesion. The correlations of CD147 and Cyp A with lesion size were evaluated.Results Lymphoid cells, epithelial cells, macrophages and fibroblasts cells showed CD147 and Cyp A positive immunostaining in the lesions. The positive expression of CD147 were detected in all periapical lesions tissues, negative expression in negative control group and less expression in healthy bones. The expression of CD147 and Cyp A in RCs was higher than in PGs( P<0.05). Positive correlations between CD147 and Cyp A protein levels were observed in PGs(r=0.787, P<0.05) and RCs(r=0.755, P<0.05), respectively. Furthermore, CD147 and Cyp A were positively correlated with lesion sizes(P<0.05).Conclusions 1. The expression of CD147 and Cyp A in RCs and in PGs was significantly higher than in control groups. CD147 may be involved in the periapical inflammation response by the experiment of immunohistochemistry analysis. 2. The expression of CD147 and Cyp A in RCs was higher than in PGs. Positive correlations between CD147 and Cyp A protein levels were observed in PGs and RCs, respectively. These findings demonstrated that CD147-Cyp A interactions might be involved in the development and progression of periapical lesion. 3. CD147 and Cyp A were positively correlated with lesion sizes. CD147-Cyp A interactions might be involved in the inflammatory response, and bone resorption of apical periapical. The two factors may play a synergistic role in the development and progression of periapical lesion.
