Objective: The purpose of our study is to demonstrate the clinical features and management of patients with ductal carcinoma in situ with microinvasion(DCIS-MI),to seek for rationale management. Methods: The clinicopathologic data of 318 cases of patients from January 1st 2004 to December 31 th 2014 were retrospectively reviewed,including 239 cases of ductal carcinoma in situ(DCIS),and 79 cases of DCIS-MI. Results: There was statistically significally relationship between DCIS and DCIS-MI with nuclear grade or with comedo-type necrosis or with ER positive or with PR positive or with Ki-67 positive or with molecular subtypes or with surgical options(P<0.05).There was no statistically significant relationship between DCIS and DCIS-MI with age,nation,menopausal,location of mass,clinical presentation(including breast lump,nipple change and asymptomatic),calcifications, size of mass and Her-2 positive(P>0.05).With a median follow-up of 66 months, DCIS group had 2 cases of chest wall recurrence,the recurrence rate is 0.8%(2/239), DCIS-MI group had 2 cases of chest wall recurrence,the recurrence rate is 2.5%(2/79),there was no difference in the recurrence rate or overall survival rate between DCIS and DCIS-MI(P>0.05). The recurrence of DCIS-MI was not related with operation methods, nuclear grade and axillary lymph node dissection(P=1.000、0.470、0.971).Conclusion: DCIS-MI is a less common type of breast carcinoma with extremely good prognosis,there was no statistically relationship between DCIS and DCIS-MI with axillary lymph node metastases and prognosis. We should treat DCIS-MI according to DCIS. Breast surgery and sentinel lymph node biopsy(SLNB) should be as DCIS-MI routine operation to avoid excessive treatment.
