Objective:To investigate the early diagnostic value and prognostic significance of serum heparin-binding protein in patients with sepsis. Methods:A total of 60 sepsis patients admitted to ICU from January 2015 to January 2016 were enrolled in the study.Sixty sepsis patients were divided into three groups according to clinical severity:general sepsis group(20 cases), severe sepsis group(20 cases) and septic shock group(20cases).In addition, according to clinical prognosis in 28 days, the patients were divided into survival group(49 cases) and death group(11 cases).Another 28 age-matched healthy cases having passed physical check-up were chosen as the healthy control group.The serum HBP level of patients in all groups was detected by enzyme-linked immunosorbent assay(ELISA), and at the same time,blood procalcitonin(PCT), white blood cell count(WBC),C-reactive protein(CRP),plasma lactic acid(Lac) and acute physiology and chronic health evaluationⅡ(APACHEⅡ) score were also determined.We compared HBP,PCT,Lac,WBC,CRP and APACHEⅡscore in all groups.Correlation analysis was studied between HBP and other indicators.We also studied the early prognostic value on sepsis mortality by establishing receiver operating characteristic(ROC)curve. Results:The level of HBP was obviously higher in case group than that in control group;Comparison of HBP,PCT,Lac,WBC,CRP and APACHE Ⅱ score in 3 subgroup of sepsis all showed significant difference(all P<0.05).The levels of serum HBP has strong linear correlation with Lac and APACHEⅡscore(r=0.178,0.358;all P<0.05).The level of HBP was obviously higher in death group than that in survival group(t=1.998,P<0.05).The area under curve(AUC)of HBP predicting 28-day mortality of sepsis patients was 0.785,with sensitivity 0.818,specificity 0.694.There was no difference of HBP level between Gram-negative baeterium and Gram-Positive baeterium and bacterium and fungus.Conclusion:Detecting serum HBP could be useful for assessment of early diagnosticvalue and severity in patients with sepsis.HBP is a good and specific evaluation marker for predicting the mortality of sepsis patients.The serum HBP level could not help differentiate Gram-negative baeterium and Gram-Positive baeterium and bacterium and fungus.
