Objective To debate the CT and MRI image features of orbital Langerhans cell histiocytosis and the value of CT and MRI in the qualitative diagnosis and localization diagnosis of Langerhans cell histiocytosis in orbit, in order to improve the preoperative diagnosis accuracy of tumors and provide more valuable information for clinical.Materials and Methods The image of CT and MRI findings in 32 patients with orbital Langerhans cell histiocytosis were retrospectively analyzed in our hospital in recent sixteen years since 2000.Through analysis of the imaging characteristic,we research differential diagnosis and infringement of craniocerebral in orbital Langerhans cell histiocytosis.The exact rate of CT and MRI in qualitative diagnosis and the positive rate of the invasion of the bone and soft tissue were calculated respectively.SPSS17.0software was used for statistical processing, and χ2 test was performed for statistical analysis,P < 0.05 was regarded as statistically significant.Results The clinical symptoms were exophthalmia.32 cases,age range was from 0.3 to 35 years old with an average of 2.5 and male to female ratio 1.67∶1.32 cases,including 22 cases occurred in the superior orbital wall(outer orbital wall), 4 cases of medial orbital wall,under the orbital wall 2 cases and more orbital walls 4cases,which were located on the left side of the 22 cases;located on the right side on the 10 case.On HRCT,the lesions characterized osteolytic destruction with large soft tissue mass clearly.Imaging of MRI demonstrate hypointensity signal on T1 WI and isointensity or hyperintensity signal on T2 WI.On enhanced CT and MRI,the lesions showed moderate to marked enhancement.The positive rate of CT and MRI the diagnosis of orbital Langerhans cell histiocytosis was 43.75% and 62.50%,no significant difference between them(χ2=2.26,P>0.05).The positive rate of CT and MRI in the diagnosis of the invasion of the surrounding bone of orbital Langerhans cell histiocytosis was 93.75% and 56.25%,there are significant differences between them(χ2=12.00,P<0.05), the positive diagnosis rate of orbital Langerhans cell histiocytosis invading the surrounding bone in CT examination was higher than MRI;The positive rate of CT and MRI in the diagnosis of the invasion of the surrounding soft tissue was 59.38% and87.50%,respectively, there are significant differences between them(χ2=6.48,P<0.05),the positive diagnosis rate of orbital Langerhans cell histiocytosis invading the surrounding soft tissue in MRI examination was higher than CT.Conclusion MRI,as an important imaging method of orbital disease diagnosis,can clearly and accurately display the scope of lesions.HRCT can present osteolytic destruction obviously.The lesion occurs mainly in children,early found early treatment help to the health and development of children,and whether a manifestation of intracranial violation of the patient prognosis has a decisive role by using MRI enhancement scanning combined with fat suppression test method,dural enhancement signs present meningeal enhancement for diagnosis.Therefore,we are committed to improve the rate of diagnosis of orbital Langerhans cell histiocytosis and estimate whether intracranial invasion accurately and provide effective basis for clinical by a combination of CT and MRI imaging information.This article is focus on the differential diagnosis of related diseases,improving the awareness and diagnosis of the disease and enhancing the cure rate.
