ObjectiveTo observe the prevalence of osteoporosis in patients with spondyloarthritis(SpA) and analysis the correlations between the bone density and bone metabolic marks. MethodsA total of 77 SpA patients from the department of rheumatology in the China-Japan union hospital of JiLin university who hospitalized during the period from 2013 to 2014 were enrolled in the study. 48 patients were diagnosed with axial spondyloarthritis and the others were diagnosed with peripheral spondyloarthritis according to the classification criteria published by ASAS in 2009 and 2010 respectively. 30 controls from medical examination center of the same hospital were enrolled. Those with diseases(such as diabetes mellitus, nephritic syndrome and disease of thyroid gland) or taking drugs(such as calcium, glucocorticoid and bisphosphonates) which might affect the BMD were excluded from the study. There was no statistical difference between patients and controls in age and sex.SPSS 19.0 software was used for results statistical analysis. Results32.4% of patients with spondyloarthritis had osteoporosis,29.9% had osteopenia,where as in control group 0% had osteoporosis, 13.3% had osteoporosis.The incidence of osteoporosis and osteopenia in SpA patients with the course within one year were 15% and 30% respectively, the course of the disease in 1-5 years were 37% and 25.9% respectively, the course of the disease above 5 years were 40% and 33.3% respectively. Comparing with the controls, the bone mineral density of lumbar spine, femoral neck, femoral trochanter and Ward’s triangle were significantly lower(p all <0.01),while the procollagen I N-terminal peptide and human type I collagen C terminal peptide were significantly higher than controls(p<0.01,p<0.01, respectively).(4)The bone mineral density of lumbar spine, femoral neck, femoral trochanter and Ward’s triangle in SpA patients showed a negative correlation with the course of the disease(p<0.05).ConclusionThere was a widely existed bone loss in SpA patients,which increased with the course of illness.Measuring bone metabolic markers might help to find bone loss early.
