Yunnan Baiyao Adjuvant Treatment Of Acute Upper Gastrointestinal Bleeding: A Meta-analysis

Background: Hemorrhage of upper digestive tract is a common and serious complication in clinic, it is urgent and the mortality is high. At present, drug treatment is still the primary treatment for upper gastrointestinal bleeding. Conventional treatment for upper gastrointestinal bleeding includes acid suppressive drugs, vasoactive drugs, promoting gastric motility drugs, antibiotics and other drugs. Hemostasis mechanisms and clinical efficacy of most drugs have been clear. Yunnan Baiyao is in Chinese traditional medicine procoagulant and promote the wound healing of the classical drugs, the existing literature proves that it can shorten the clotting time and promote the role of platelet aggregation and antibacterial, anti-inflammatory effect, domestic large clinical trials prove it in treatment of upper digestive tract hemorrhage in good effect, but there is no literature of Yunnan Baiyao in the treatment of hemorrhage of upper digestive tract were evaluated.AIM: To system review Yunnan Baiyao auxiliary conventional drugs in the treatment of digestive tract hemorrhage curative efficacy and safety, providing strong evidencefor the clinical application.METHODS: Computer retrieval Pub Med, CNKI, VIP, CBM and Wanfang data, from inception to 2015 September on the Yunnan Baiyao in the treatment of upper digestive tract hemorrhage randomized controlled test(RCT). The R3.2.3 statistical software to research into the Meta-analysis and Yunnan Baiyao in the treatment of digestive tract bleeding, the total effective rate and significant efficiency as the main evaluation index, combined with the average bleeding time, blood transfusion and adverse reactions to the Yunnan Baiyao curative effect of comprehensive evaluation, the summary odds ratio(or) and 95% confidence intervals(95% CI) were calculated, and the index of sensitivity analysis and publication bias test.RESULTS: A total of 22 studies were included in the rigorous screening. The total efficiency into 21 studies.(or = 4.92, 95% CI(3.65,6.63), P< 0.01); effective rate into the 21 studies(or 2.35, 95% CI(1.96, 2.82); average hemostasis time into 9 studies(SMD =-3.05, 95% CI(-4.03, 2.07), P< 0.01); blood transfusion into 5 studies(SMD =-3.13, 95% CI(-3.41,-2.84), P< 0.01); adverse reactions included 5(or = 0.75, 95% CI(0.41, 1.36), P= 0.338 > 0.05) study. Total effectiveness andeffect of Yunnan Baiyao auxiliary routine medication in the treatment of acute hemorrhage of upper digestive tract were significantly higher than that of the control group; average bleeding time and volume of blood transfusion were significantly less than the control group, adverse reactions and control groups did not differ.Conclusion: Yunnan Baiyao auxiliary routine medicine in treating digestive tract hemorrhage curative effect and safety of good, can be used in clinical practice.
