Background:Currently with the high incidence of lung cancer worldwide, more and more scholars pay attention to respiratory system disease complicated with lung cancer,lots of researches on clinical more common interstitial lung disease complicated with lung cancer and chronic obstructive pulmonary disease complicated with lung cancer,but for bronchial asthma complicated with lung cancer reported less, and there are a lot of controversy, and bronchial asthma as a common and frequently occurring disease of respiratory system, whether between lung cancer and associated, still need to be further explored.Objective :To sum up, summarize and analyze the clinical features of bronchial asthmatic patients complicated with lung cancer, explore the relationship between bronchial asthma and lung cancer, provide some help for early detection and diagnosis of lung cancer.Methods:Retrospectively analyzed the clinical characteristics of 19 bronchial asthmatic patients complicated with lung cancer treated by the second hospital of Jilin university from 2009 to 2015.ResultsThe incidence of bronchial asthma combined with lung cancer accounted for1.4% of the patients with bronchial asthma in our hospital,accounting for 1.1% of lung cancer patients in our hospital.All patients were older than 40 years old,11(58%) patients were over 60 years old,cough and expectoration were the main clinical symptoms of 14 cases(74%). From the time of diagnosis of asthma,13 caseswith definite diagnosis of asthma, 10 years or more in 8 cases(42%).From the pathological type,4 cases of squamous cell carcinoma(21%),7cases of small cell carcinoma(37%),7cases of adenocarcinoma(37%),1case of poorly differentiated non-small cell carcinoma(5%).In small cell lung cancer, 5 cases of limited stage, 2cases in a wide range. In non small cell lung cancer,2cases were stage II, 2 cases with stage Ⅲ a, 7 cases with stage Ⅲ b, 1 cases with stage IV. From chest CT performance, the main manifestations are nodular, patchy, and mass like. All patients had no effective control of asthma. In 10 patients, the PS score was poor and had not been treated with anti tumor therapy.9 cases were treated with anti tumor therapy.Conclusion1.Epidemically period of asthma was in childhood and old age,while peak period of lung cancer was in old age.The older are majority in patients of asthma complicated with lung cancer. 2.Although the incidence of asthma complicated with lung cancer was not high, patients in asthma exacerbation were easily diagnosed without lung cancer. Therefore, for athma patients or older than 60 years patients,the first suggestion was effective control of asthma attack, according to the guide for prevention and treatment of bronchial asthma updated 2008 of asthma group of Chinese thoracic society. Once new symptom of cough, sputum, breathing, chest pain, blood in phlegm and others appear, especially infection without explanation,regular chest CT scans or low doses of chest CT examination per year were beneficial to the early diagnosis of asthma complicated with lung cancer patients. Meanwhile,for the first-diagnosis older asthma patients for the first time,actively elimination of tumor possibility was still needed to avoid misdiagnosis.3.The pathogenesis of asthma complicated with lung cancer was unclear, which might be associated with inflammatory stimulation. 4.Treatment of lung cancer in patients with severe asthma was still a clinical problem to be solved.
