ObjectiveBased on the trans-theoretical model making an health education plan for starting stroke patients recovery.To analyze the level of stroke knowledge and self-management behavior in patients with first stroke.To explore the effect of health education based on trans-theoretical model (TTM) on abilities of daily life、the level of stroke knowledge and self-management behavior.It will provide a reference basis to formulate strongly handle、 good effect health education model in the future.MethodsThis research is divided into two parts. The first part:formulate the trans-theoretical model of health education plan, through literature study, expert consultation method and preliminary experiments established based on the trans-theoretical model of health education programs.The second part:clinical studies.This part selected the standard of 82 cases of first stroke patients in May,2015 to February,2015 at a level of first-class hospital in Harbin.These patients were randomly divided into intervention group (41 cases) and control group (41 cases).The control group practice routine health education,and intervention group practice the trans-theoretical model of health education.This health education program is on the basis of literature review, by experts consultation,ultimately established after the preliminary experiments.Assessing the patients’stage of behavior change before intervention,then to match the phase change of the trans-theoretical model of health education, and to give 3 months of follow-up. To evaluate the two groups of patients’daily activity ability, knowledge level and self-management behavior of stroke at before the intervention(To),after the intervention 1 month (T1) and after the intervention 3 months (T3). All collected data were typed in SPSS17.0 (Chinese version) using descriptive analysis, x2 test, two independent sample t-test, repetitive measure analysis of variance, non-parametric test methods such as statistical analysis, inspection level alpha=0.05, p<0.05 for statistically significant.Results1.The level status of the first stroke in patients with stroke knowledge:The score of stroke knowledge was(15.24±4.65).The score of each dimension in descending order were: diet, risk factors, sports, life daily life, blood pressure measurement,medication, stroke management and stroke sign.2.The level status of the first stroke in patients with self-management behavior:The score of self-management behavior was (141.96±14.97).The score of each dimension in descending order were:drug safety management,dietary management,emotion management, social function and interpersonal management,life daily life management, rehabilitation management and disease management.3.Before the intervention, two groups of patients in the general information, daily activity ability and the level knowledge of stroke, the total score of self management behavior and each dimension score was no statistically significant difference(p>0.05),had comparability.4.Daily life activities ability of intervention effect assessmentDaily life activities ability after intervention in 1 month,3 months after the intervention, the comparison between the two ways without statistical significance (p> 0.05); Daily life activities to improve both the two groups after intervention, each time point in group comparison difference was statistically significant (p< 0.05)5.Levels of knowledge of stroke intervention effect assessmentLevels of knowledge of stroke after intervention in 1 month,3 months after the intervention, intervention group stroke knowledge scores were significantly higher than that of control group, with statistical significance (p< 0.05); The level of knowledge of stroke was improved, the two groups after intervention each time point in group comparison difference was statistically significant (p< 0.05).6.The total score of self-management behavior and each dimension of intervention effect assessmentSelf-management behavior score after intervention in 1 month,3 months after the intervention, in addition to the drug management and food management, self-management behavior intervention group total score and each dimension scores are higher than the control group, with statistical significance (p< 0.05); Intervention group in addition to the diet dimension, total score and each dimension scores are improving, each time point in group comparison had statistical significance (p< 0.05); Compared with the control group after intervention in 1 month before the comparison, in addition to the diet management have statistical significance (p< 0.05); 3 months after intervention in 1 month after the intervention, disease management, diet, daily life management and rehabilitation management four dimensions has no statistical significance (p> 0.05), and the rest was statistically significant (p< 0.05).3 months after intervention in comparison, compared with before, the total score of self-management behavior and each dimension scores were statistically significant (p< 0.05)Conclusions1.First stroke in patients with stroke knowledge score was in the middle rdi level,remains to be improved.2.First stroke in patients with self-management behavior score was in the middle rdi level, remains to be improved.3.Two kinds of health education scheme can improve the ability of the patient’s daily activities, and intervention effect did not see the difference.4.Trans-theoretical model of health education scheme can effectively improve the level of knowledge of stroke patients, and the effect is better than that of routine health education.5.Trans-theoretical model of health education can effectively improve the level of self management behavior, the long-term effect is remarkable, is superior to routine health education.
