Objective : To estimate the clinical short-term effects of several compression screwsinternal fixation(IF), total hip arthroplasty( THA) and femoral head arthroplasty( FHA) in the management of displaced neck fractures in the elderly patients.Method: From February 2008 to October 2013,we retrospectively analyzed the clinical data of 70 cases of displaced neck fractures. The patients were divided into several compression screws internal fixation(group A, n=20),total hip arthroplasty(group B,n=27)and femoral head arthroplasty(group C, n=23).The operation time,intraoperative blood loss and postoperative bed time, health service costs, complications after operation were compared. Clinical outcomes were measured with using Harris scores during 1-3 year of follow-up.Results: Compared with group B and C,the operation time and intraoperative blood loss were significantly less(P <0.05)in group A. Compared with group B and C, the postoperativebed time was markedly longer(P <0.05)in group A, however,the rate of complications after operation were highest in group A( P <0.05), but there was no significant difference between group B and group C(P > 0.05). According to health service costs, showing significant differences among 3 groups(P < 0.05),Compared with group A,the excellence rate of postoperative Harris scores were remarkably higher(P <0.05)in group B and C.but there was no significant difference between group B and group C during1-2 years of follow-up(P > 0.05).However, compared with group C,the excellence rate of postoperative Harris scores were remarkably higher(P <0.05)in group B during 3 years of follow-up.Conclusions :Arthroplasty is more clinically effective than several compressionscrews internal fixation in the elderly patients with a displaced intracapsular fracture of the hip. According to patient’s age, life expectancy, general and financial conditions, THA is a reasonable choice for the elderly patients with displaced femoral neck fracture.
