Screening And Researches On The Relative Prognostic Markers Of Colorectal Cancer

Objectives:To search for the prognosis related markers of colorectal cancer,to explore its application value in the prognosis of stage III colorectal cancer, to establish a model to predict prognosis and provide guidance for clinical diagnosis and treatment.Methods:In this study, a comprehensive analysis of molecular markers for the prognosis of colorectal cancer has been studied.Five molecular markers associated with the prognosis of colorectal cancer were established through screening(CA4, MSH2, Osteopontin, SPARCL1, ANKRD12).We enrolled 361 patients who have undergone stage Ⅲ colorectal cancer surgery in the second affiliated hospital of Zhejiang University between the year 2001 and 2011.Paraffin-embedded specimens of the patients were collected to prepare tissue micoarrary.Immunohistochemical analysis was performed to detect the expression level of the markers in tumor tissues.The relationships between each candidate molecular marker and clinical pathological parameters and prognosis were evaluated. Logistics regression model was used to established prognosis related model and the ROC curve was adopted to evaluate the model’s accuracy.Results:The expression level of CA4, MSH2, Osteopontin, SPARCL1, ANKRD12 in stage III colorectal cancer tissue was not significantly correlated with gender and age of patients (P>0.05).Markers significantly related with 3 year survival rate were CA4^ ANKRD12, while for 5 year survival rate the marker was ANKRD12.Survival analysis of these 5 markers using Kaplan-Meier survival analysis showed that CA4、MSH2、 ANKRD 12 were significantly correlated with prognosis of stage III colorectal cancer(P<0.05). The 5 markers were incorporated into the Logistics regression model, and the accuracy of the model was judged by ROC curve. Taking 3 years survival ratio as a judgment index, the results showed that the expression levels of CA4, Osteopontin, MSH2, SPARCL1, ANKRD 12 can determine the prognosis of patients with stage III colorectal cancer (AUC=0.713).Similarly,taking 5 years survival ratio as the judgment index,CA4, MSH2, Osteopontin,, SPARCL1, ANKRD 12 can determine the prognosis of patients with stage III colorectal cancer (AUC=0.726).Conclusion:Although the expression level of CA4, MSH2, Osteopontin, SPARCL1, ANKRD12 in stage III colorectal cancer tissues was not associated with gender and age of the patients,it could suggest the prognosis of patients with stage III colorectal cancer.
