Objective:Describe the operation status of severe abnormal patients or clusters surveillance project; analysis the characteristics, main problems and reasons of severe abnormal patients or clusters surveillance project; evaluate the Attribute and index of severe abnormal patients or clusters surveillance project; to found the main problem, get specific measures and methods; to provide suggestions for improving the efficiency and quality of the project, and improve the project.Methods:Refer to the guidelines of disease monitoring and evaluation released by the and the infectious disease surveillance and response system of monitoring and evaluation framework set by WHO, in combination with the characteristics of monitoring pilot project, through the three independent design method of investigation (the survey of running status self-evaluation from the pilot units, the questionnaire survey of the KAP project staff and the retrospective survey of the case record),we investigated and assessed the operation situation, the results and evaluating indexes of the monitoring the pilot project on September 1 to November 30,2014.Results:1. The operation situation1.1 There were 20 project pilot from Shanghai and Jiangsu Province, which included 11 and 9 hospital, carried out the monitoring of severe clinical abnormal cases/event except Nanjing children’s hospital until August 31,2014.1.2 The of Minhang District of Shanghai, Nanjing and Suzhou city didn’t carry out the project training in 2014.1.3 Nanjing Drum Tower Hospital and the Wujing hospital of Minhang District didn’t carry out the quality control until August 31,2014.1.4 The of Nanjing and Minhang District didn’t carry out active surveillance.2. Surveillance results2.1 The monitoring projects grabbed or disappeared the 262 cases of suspected serious clinical cases and 42 cases of abnormal suspicious clinical cases until September 30,2014.Upon examination, there were no cases and incident reports in Shanghai. There were reported and disposed of 37 severe clinical cases, including 2 severe clinical gathering incidents.2.2 The result of the retrospective survey of the case record showed the reporting rate of severe clinical abnormal cases was 66.7%.3. Evaluating indexes3.1 PracticalityThrough the monitoring of the serious abnormal cases and clinical, we recognized to find the new emerging infectious diseases, eg:2 human infection with H7N9 virus.3.2 SensibilityThe discovered severe clinical abnormal cases accounted for 66.7% of the actually.3.3 The quality of monitor dataThere was no monitor data, error score was 0%.3.4 TimelinessThe median was 10 days (0-24 days) between the onset and report time in the monitoring pilot project; the median was 1.5 days(1-2 days) between the onset and report time in event indicator cases.3.5 FeasibilityThere were 124 staff (59%) approved the monitoring project; the action rate was 51%;there were 104 staff thought the monitoring increased the workload; there were 93.6% staff accepted the simplicity of the project design; there were 74.2% staff accepted the simplicity of the definition of the monitor cases; the current staff of the pilot unit could satisfied the demand of monitoring work basically.Conclusion:We found that through the different reporting of the pilot project could achieve the purpose of early Recognizing and detecting the emerging infectious diseases and severe abnormal patients or clusters, which has a strong general, applicability and feasibility.The reporting ways of the Shanghai City and Jiangsu Province were different, but Can be carried out. The model of the WuJing hospital can be conducted in the areas with high level of economic and health information, the model of the first people’s Hospital of ChangShu city can be conducted in the areas with low level of economic and health information.
