Objective:Studies of the expressions and changes in the expression of serum immunoglobulin λ light chain(λ-Ig LC), filaggrin(FLG), transthyretin(TTP) in the serum of chinese adult Han patients with obstructive sleep apnea hypopnea syndrome(OSAHS), and the relationship between the expression and OSAHS severity,acute cerebral infarction,acute upper respiratory infection and PSG parameters.It provides a new method and approach for OSAHS clinical screening, and provides a parameter for polysomnography Figure(PSG).It will establish a database to provide baseline data for the prospective study.Methods:50 cases who were inpatient and outpatient and 10 cases of healthy people were included from July 2013 to December 2015 in the No.1 Hospital of Jilin University, firstly performed polysomnography,and then according to PSG results and medical history, the subjects were divided into the control group(whose apnea-hypopnea index<5, AHI<5),OSAHS group(AHI≥5), acute cerebral infarction combined OSAHS group(AHI≥5), acute upper respiratory infection group(AHI<5). 10 cases were included into the control group(age = 59.8±11.4 years); 30 patients were included into the OSAHS group(age = 50.5±12.4 years), which then was further divided into three sub-groups according to the severity of OSAHS, that is: light group(5≤AHI≤15),moderate group(15 <AHI≤30),and serious group(AHI> 30), with 10 patients in each group;10 cases were included into the acute cerebral infarction combined OSAHS group(age = 49.7±11.8 years);and 10 cases were included into acute respiratory infection group(age = 49.0±16.5 years). Collecting their medical histories, and collecting their blood samples, aliquoted and frozen at-20 ℃ refrigerator. Thaw and then centrifuged to remove the sediment.Biotin double-antibody sandwich enzyme-linked immunosorbent assay(ie ELISA) is apllied for detection of serum λ-Ig LC, FLG, TTR in the Northeast Han adult population, and the application of SPSS19.0 statistical software for statistical analysis. Results:1. Control group, OSAHS group,CI group,infection group were the presence of the expression of λ-Ig LC, FLG, TTR, and its values are as follows: Control: λ-Ig LC2.859(1.174-3.545),FLG(1.890±0.927),TTR(2.266±1.666); LOSAHS: λ-Ig LC(4.187±2.446),FLG 2.256(1.746-6.381),TTR(2.984±2.440); MOSAHS:λ-Ig LC(4.431±2.793),FLG3.991(2.013-7.806),TTR4.609(1.764-9.294); SOSHAS:λ-Ig LC1.933(1.181-3.210),FLG3.34(2.110-6.144),TTR(4.053±2.564); CIOSAHλ-Ig LC(7.801±2.826),FLG8.467(5.266-9.162),TTR10.091(3.996-13.080); UBI: λ-Ig LC2.485(1.784-4.393),FLG(3.075±0.402),TTR(5.724±2.764).2. Analysis of differences in serum protein λ-Ig LC, FLG, TTR expression displaied that: compared with the control group, the expression difference of λ-Ig LC, FLG, TTR in OSAHS group was not statistically significant(P> 0.05),while the expression difference of λ-Ig LC, TTR in cerebral infarction group was statistically significant(P <0.05); Compared with OSAHS group, the expression difference of λ-Ig LC, FLG, TTR in cerebral infarction group was statistically significant(P <0.05); the expression difference of FLG between LOSAHS group, MOSAHS and the CI group was statistically significant(P <0.05); the remaining pairwise comparisons difference between groups was not statistically significant.3. Correlation studies between OSAHS grade and BMI of population in the Northeast displaied that BMI was related to the OSAHS severity(P<0.05); Correlation studies between BMI and λ-Ig LC、FLG、TTR showed that BMI was not related to the concentration of λ-Ig LC、FLG、TTR.4. We do the evaluation of diagnostic tests curves of λ-Ig LC, FLG, TTR, respectively, in the control group and the experimental group,and area under the curve(AUC) showed that: λ-Ig LC in OSAHS group, cerebral infarction group, infection group AUC were 0.406,0.858,0.511, respectively sensitivity were 0.917,1.000,1.000, and specificity were 0.100,0.558,0.295; FLG in OSAHS group, cerebral infarction group, infection group AUC were 0.204,0.867,0.591; sensitivity were 0.750,0.545,1.000 and specificity were0.222,1.000,1.000; at TTR in OSAHS group, cerebral infarction group, infection group AUC were 0.354,0.825,0.605, sensitivity, respectively, 1.000,0.636,1.000, specificity were 0.033,0.907,0.500. Conclusion:1. The expression of the protein λ-Ig LC, FLG, TTR are the presence of serum in both control group and test groups;2. The expression levels of serum protein λ-Ig LC, FLG, TTR in CIOSAHS group increased.3.There is a correlation between BMI and the serverity of OSAHS in patients;4. λ-Ig LC, FLG, TTR can not replace PSG to diagnose OSAHS, but associated with a diagnosis of cerebral infarction combined OSAHS.
