Background and objective : Breast cancer is the most common malignant tumor in women in the world, and it seriously damage female’s physical and mental health. While in China, the morbidity and mortality of breast cancer increased annually. As to the improvement of screening technology, which subsequently lead to the early detection of breast cancer patients, and tailored comprehensive treatment to each breast cancer victims, the overall survival of breast cancer patients has been improved dramatically. Breast mass found by physical examination or imaging methods is a common cause of female medical treatment, and also the most common clinical symptoms of breast cancer. How accurate the screening methods of breast cancer patients are is the key to early diagnosis of breast cancer. Physical examination, imaging detection and pathology evaluation are the most commonly used in clinical practice to assess whether the breast tumor is malignant or not. But pathological diagnosis of breast tumor is the final decision which of high accuracy. In clinical practice, there are several ways to obtain patients’ sample including fine needle aspiration biopsy, core needle biopsy(CNB) and vacuum needle biopsy. Among all those procedures CNB is the most widely used to determine whether further surgery need to be performed or not. But the results of the CNB can be affected by many factors and the accuracy of CNB in each clinical hospital is different. This study evaluated the accuracy of the breast cancer diagnosis by the CNB in our hospital, and discussed the safety, side effects and economic benefits of CNB.Methods: Statistics all patients’ basic information, auxiliary imaging reports, CNB results, late treatment and final pathological diagnosis who had the core needle biopsy of breast tumor in the breast surgery of the second hospital of Jilin University from January 2014 to December 2015. By using the SPSS20.0 statistical software package for data processing, the same patient’ results of the CNB and resection were matched to the data.Results: There were 583 patients who had the core needle biopsy of breast tumor, of which 4 patients puncture 2 breast tumors, a total of 587 cases in the breast surgery of the second hospital of Jilin University from January 2014 to December 2015. Depend on the report of CNB, 161 cases were benign and 426 cases were malignant.After the CNB examination,479 breast tumor had resection.The accurate rate of CNB was 98.75%(473/479), the sensitivity of malignant breast tumor was 98.36%(403/409), the specificity was 100%(70/70), the positive predictive value was 100%(403/403),value of the negative predictive was 92.1%(70/76), the misdiagnosis rate was 1.47% 6/409 and the the rate of underestimate was 1.22%(5/409).The consistency of CNB and surgical resection was strong(Kappa=0.9515). There were 6 breast tumor got misdiagnosis of breast cancer, 5 of which havingimaging diagnosis of intraductal tumors(1 cases was fibroadenoma, 4 cases were intraductal papilloma by CNB), 1 cases was atypical ductal hyperplasia.Conclusion: 1.The CNB guided by the color Doppler ultrasound has small trauma, simple operation, high accuracy of pathological diagnosis, surgical resection and pathological diagnosis results of high consistency,and it is accurate, safe, economical and effective in the diagnosis of breast tumors.2.Intraductal papilloma,atypical ductal hyperplasia and carcinoma in situ are easily got misdiagnosis or underestimated by CNB, they need further surgical biopsy to confirm the diagnosis.
