PTK For The Treatment Of Corneal Dystrophy

Objective:To investigate the treatment of corneal optical ablation(phototherapeutic keratectomy, PTK) curative effect in the treatment of superficial corneal dystrophy.Methods:Collected from September 2009 to March 2016 in our hospital, 24 cases of 13 patients with superficial corneal dystrophy were hospitalized. Among them, 9 cases(16 eyes) and 4 cases(8 eyes) were female. 2 eyes, 11 eyes. The age ranged from 21 to 60, with an average of 43 years. The age of onset was 13 years old, the maximum was 46 years old, and the average age was 24.1 years. History of 5 years to 35 years, an average of 15.9 years. In 13 patients, crystalline corneal dystrophy 1, macular corneal dystrophy, 2 people, 10 people of lattice corneal dystrophy. Ranging from 0.02 to 0.3, preoperative visual acuity, including 0.02-0.04(1 eyes), 0.05-0.08(9 eyes), 0.1-0.25(13 eyes), 0.3-1.0(1 eyes). All of the 24 eyes with phototherapeutic keratectomy. The operation uses Allegretto wave eye-q excimer laser system PTK treatment was performed by the same physician. Before operation, patients need to use levofloxacin eye drops eye drops, the frequency was 6 times daily for 3 days. Based on the preoperative eye with the preoperative refraction, scar depth, the extent of the lesion to set the parameters of the laser, the technical parameters of input computer. Physiological saline flushing eyes, Austrian hydrochloride Buka for eye drops(Connaught times like) surface anesthesia for 3 times, regular disinfection drape, eyelashes paste, eyelid eyelid opening. Determine the corneal optical center and the center of the pupil, enjoin patient the eye does not move after, according to the preset amount of emission laser corneal surgery. The energy density of 160 m J/cm2, single pulse frequency 10 Hz, cutting depth of 0.25 um, diameter of 5 ~ 7.5mm. Continuous laser pulse, the cutting depth is 50 ~ 100 fzm. At the end of surgery when placed soft corneal contact lens before operation after operation by slit lamp examination, oculyzer anterior segment analysis system, refraction, intraocular pressure, contrast sensitivity test, ultrasonic corneal thickness, tear film examination, dilated fundus examination. The changes of visual acuity were observed after surgery, indications, treatment parameters and postoperative complications. The follow-up time was 6 months to 36 months, an average of 15 months. To summarize the treatment of corneal optical cutting postoperative clinical symptoms and improve the recovery of visual acuity and complications, the clinical effect of treatment of superficial corneal dystrophy.Results: 24 eyes of 2 patients(4 eyes) had delayed healing after surgery, the visual acuity of the patients with transient decrease of visual acuity was improved after surgery. The best corrected visual acuity 0.1 in 24 eyes(100% literacy rate). The best corrected visual acuity 0.3 in 14 eyes(58.3%, residue removal rate). Preoperative and postoperative visual acuity compared by 2 test, there was significant difference(P<0.01).
