Objective:To investigate the efficacy of transcallosal-interforniceal approach for craniopharyngioma resection, to explore the advantages and disadvantages of this approach, and improve the therapeutic effect of craniopharyngioma. Methods:Summarize the clinical data of 21 cases of patients,selection of operative approach, postoperative complications and long term outcome of patients. In this paper, all cases were treated by surgical treatment under general anesthesia with tracheal intubation. All patients were divided into two groups, the first group of 11 cases were selected by pterional approach, the second group of 10 cases were selected by transcallosal interforniceal approach. Results:The postoperative complications of the two groups included short time consciousness, water electrolyte disorder, epilepsy and low pituitary function, In the second group, 2 cases with serious short-term memory loss, 1 case with serious mental symptoms. Conclusion:When the tumors occurred in the optic chiasm rear, intruding into the third ventricle or associated hydrocephalus, the transcallosal-interforniceal approach can expose the tumor more fully, beneficial to total resection. However, there are some serious irreversible complications such as memory disorders and psychiatric symptoms in the long term outcome.
