Objective:In recent years China’s medical disputes and there is a trend of increasing year by year cause the highbound, medical staffs working environment is relatively worse, gradually strained doctor-patient relationship, and how to reduce medical disputes and to build a harmonious relationship between doctors and patients is a problem that must be solved in the development of medical and health undertakings in China, this study of Shandong Province a large three level of first-class comprehensive hospital obstetrics and gynecology medical malpractice cases research, mainly studies in the Department of Obstetrics and gynecology medical disputes characteristics and controversial aspects of improve quality of care and services, prevention and mitigation measures of medical disputes in obstetrics and gynecology.Methods:Computer retrieval CNKI (CNKI:2010~2015.9), Wanfang database 2010-2015.9 VIP database 2010~2015.9, ProQuest, MEDLINE and other online databases collected at home and abroad about the medical dispute of literature, and combined with the related data of Shandong Province, a large three level of first-class comprehensive hospital obstetrics and gynecology medical disputes. This study mainly on a large tertiary general hospital in Shandong Province in 2005 January to December 2014 147 women obstetric medical dispute cases were retrospective analysis, using descriptive statistical methods of medical dispute in distribution, at the same time discharge number, the same bed turnover number, age, to question link and medical dispute solving ways were analyzed and a summary of the various data and the relationship between the occurrence of medical disputes.Results:There were 147 cases in the hospital from 2005 to 2014, and the average rate of the hospital obstetrics and gynecology medical disputes was 0.84/million. Medical disputes during hospitalization were significantly more than that of outpatients (91.16% vs 8.84%); 86.4% patients were among the 8 cases,5.44% cases,9.52% cases,14 of them, and the other 31.97%, and the dispute resolution was the highest; the medical dispute resolution (87.76%) and the 10.88%(6.80%) were the most frequent. The dispute was mainly in the medical dispute resolution. The main reasons for the medical disputes are patient, hospital and social factors.Conclusion:Medical disputes occur mainly in the medical service of the inpatient department; the age of patients is the influence factor of medical disputes. The younger the younger, the higher the risk of medical disputes, the lower the doctors’ practice ability and the business level, the higher the risk of medical disputes, the higher the number of ward beds, the higher the risk of medical disputes. Medical workers’ diagnosis, operation, communication, service attitude, management and so on are the main links of medical disputes, including diagnosis and treatment, operation and operation of medical disputes. Analysis of medical disputes are the main reasons:one is enhanced awareness of the legal rights of patients, the patients lack of medical knowledge, medical risk consciousness of the patients is low; second is the operational skills and sense of responsibility of medical staff is low, weak sense of service of medical staff, hospital management omissions; three is social media of the hospital medical disputes of the excessive publicity, the defects existing in the national health service system, national legal system and medical insurance is not perfect and so on. Obstetrics and gynecology as a medical high risk discipline, the need to continuously improve medical technology, strengthening the medical safety and risk management, pay attention to the health education and risk prevention of patients, to promote the prevention and treatment of diversified disputes mechanism.
