Observation Of The Clinical Effect Of Ischemic Colitis Treated By Buyang- Huanwu Decoction Combined With Chang Le NO.1 Retention Enema

Objective:This topic research of the clinical curative effect of ischemic colitis by bu yang huan wu decoction combined with Chang Le NO.1 Retention enema. Explore the mechanism of traditional Chinese medicine in treating ischemic colitis and objective evaluate the effectiveness and safety.Method:Selecte 40 patients of ischemic colitis(nongangrenous), randomly divide into treatment group and control group(n=20). Both of the two groups are asked to stay in bed, monitore of vital signs as well as liver and kidney function. The two groups need to be given the western medicine convention treatment, including fasting, rehydration, maintain water and electrolyte balance. In severe cases treat with parenteral nutrition. During the treatment, we stop the drugs which may affect the curative effect. Observe the main symptoms, fecal occult blood and the change of colonoscopy before and after treatment.Treatment group: Based on general treatment of western medicine, give buyang huanwu decoction through the mouth:astragalus membranaceus 50-100g、macrocephala 15g、angelica 10g、salvia miltiorrhiza 10g、rhizoma chuanxiong 6g、red peony root 15g、peach kernel 10g、flos carthami 10g、lumbricus 9g、leech 6g,and cooperate with Chang Le NO.1 retention enema. During fasting, the patients should be given retention enema treatment by "Chang Le NO.1", 100 ml each time, qd. After patients resume oral intake, Oral buyang huanwu decoction, 150 ml each time, bid, continuous to use 3 weeks.Control group: Based on general treatment of western medicine, give Tinidazole and Sodium Chloride Injection 100 ml, Dextran Amino Acid Injection 500 ml,ivgtt,qd. Besides Intramuscular injection compound danshen injections 10 ml,add 5% glucose solution 100 ml. qd. 3 weeks one course.Results:1.In treatment group,14 cases are markedly improved, 5 cases are effective, 1 has no effect. The total effective rate 95%;in Control group, 10 cases have marked effect, 8 cases are effective, 2 have no effect. The total effective rate was 90%. Clinical curative comprehensive effect of treatment group is better than that of control group, the efficiency is also better than that of control group.2.After treatment, the TCM symptoms total integral of two groups are improve. Compared with before treatment, the significant is obviously difference(P<0.01).In the bloody stool, abdominal pain and fatigue, the effect of treatment group is better than the control group(P<0.05)3.After treatment, the colonoscopy and defecate occult blood review of two groups are improved. Compared with before treatment, the differences are statistical significance(P<0.05), and the curative effect treatment group is better than that of control group(P<0.05).4.The Ischemic colitis always occurs in the elderly over the age of 55, Females are more affected than males; it is common occurs in left half colon(70%), splenic flexure of colon(40%), descendingcolon(30%), transverse colon(25%), ascending colon(2.5%).It almost do not involve in rectum.5.During treatment, both of the groups do not have obvious adverse reaction, before and after the treatment are not found obvious abnormality in laboratory tests, such as the blood routine, urine routine, electrocardiogram, hepatic and renal function.Conclusion:Chinese medicine bu yang huan wu decoction combined with Chang Le NO.1retention enema in treating ischemic colitis is better than western medicine treatment group in improving symptoms, intestinal mucosa lesions and clinical curative effect, and the total effective rate is higher than western medicine therapy group.
