ObjectiveThe purpose of this study is to construct the discharge planning suitable for China,can provide continuity of care for patients with coronary stent implantation,and evaluation of application effect of the discharge planning mode in clinic.MethodsThis study based on the investigation in patients undergoing coronary artery stent implantation of extended care needs, Through the literature retrieval, Analysis of discharge planning guide foreign and nursing care of patients with coronary heart disease intervention of evidence-based guidelines, Preliminary construction plan of discharge for patients undergoing coronary artery stent implantation, According to expert opinions and pre experiment to revise the plan. According to the inclusion criteria, Select 106 coronary stent implantation patients from a hospital in Chongqing as the research object By convenience sampling method, The subjects were divided into intervention group and control group by random, Each group of 53 cases. The control group received routine nursing care in Department of cardiology, Starting from the second day after coronary stent implantation, The intervention group received routine nursing in Department of Cardiology, also accept discharge planning services. At the beginning, collect general information and related physiological indexes of patients,6 months after discharge, Collection indexes of length of hospital stay, cost of hospitalization, readmission rate, the incidence of cardiovascular events, the relevant clinical indexes, compliance behavior and quality of life.ResultsThe success of the 101 patients were followed up,51 cases in the intervention group,50 cases in the control group. The patients in the intervention group re hospitalization rate and the incidence rate of cardiovascular events were lower than that of the control group (P< 0.05), in the aspect of quit rate, weekly frequency of coronary heart diseaserehabilitation, two preventive drug application rate and blood glucose, self monitoring of blood pressure, Patients in the intervention group were higher than that of the control group (P<0.05), in the aspect of left ventricular ejection fraction and blood lipids and the quality of life, The intervention group were better than that of the control group (P<0.05) There were no significant difference between two groups in hospitalization expenses and hospitalization days (P> 0.05)ConclusionsBased on the evidence-based basis, Built in patients undergoing coronary artery stent implantation of discharge planningservices, can reduce the incidence of cardiovascular events and readmission rate of patients undergoing coronary artery stent implantation, can improve the related physiological indexes of patients and improve their compliance behavior and quality of life. Therefore, to achieve a smooth transition from hospital to the family, the community for patients undergoing coronary artery stent implantation.
