| Objective: The aim of this study was To study the genotype distribution of CYP2C19 in Han and Uighur Chinese population with cardiovascular and cerebrovascular diseases, the allele and metabolic phenotype frequency were compared between them,and explore the rational use of clopidogrel.Methods: The study includes 1020 patients with cardiovascular and cerebrovascular diseases at first people’s Hospital of Kashi District, in Xinjiang(November 2010 May until November 2011 July). According to the divided the patients of different nationalities of Han and Uygur subjects group by group test. Han in test group 220 cases; the Uygur subjects group 800 cases.Take two ethnic groups tested peripheral blood 2ml, according to blood genomic DNA extraction kit instructions to extract genomic DNA, c DNA microarray technology to detect two ethnic study population CYP2C19 genotype and allele frequencies.Results: A total of 6 different CYP2C19 genotypes, and in the case of Uighur test group detected a rare clinical work homozygous mutant CYP2C19*3/*3 genotype. 1 Han and Uighur test group CYP2C19 genotype test group subjects Uighur groups have significant difference(χ2=39.83, P<0.01), Han test group CYP2C19*1/*1 frequency(0.4272) is less than(0.6213)(χ2=26.42, P<0.01), CYP2C19*1/*2 frequency(0.4046) higher than Uighur test group(0.2825)(χ2=19.58, P<0.01), CYP2C19*2/*2 frequency(0.1137) higher than Uighur test group(0.0388)(χ2=18.64, P<0.01). 2 Han and Uighur test group test group CYP2C19 allele frequencies were significantly different(χ2=42.28,P<0.01). CYP2C19*1 for the wild-type allele, you can see the CYP2C19*1 allele frequency in the two ethnic groups were tested largest proportion, but Uighur test group CYP2C19*1(0.7869) high Yu Han test group(0.6454)(χ2=37.45,P<0.01), CYP2C19*2(0.1837) lower than the Han test group(0.3273)(χ2=42.16,P<0.01). CYP2C19*3 subjects between the two ethnic groups did not differ(χ2=0.054, P=0.816). 3 Han intermediate metabolizers test group(43.64%), followed by the strong metabolic type(42.72%). Uighur group subjects homozygous wild-type CYP2C19*1/*1 the highest percentage(62.13%). Han intermediary metabolism type test group(43.64%) was higher than Uighur test group(33.13%)(χ2=8.338, P<0.05), poor metabolizer type(13.64%) is higher than the Uighur test group(4.76%)(χ2=22.831, P<0.01). 4 With different gender, age, body mass index group compared between groups, these differences(between genotype groups, P>0.05) was not statistically significant.In conclusion: 1 Han and Uighur test group test group in CYP2C19 genotype, allele distribution and gene phenotype distribution statistically significant. 2 Shows the results of the test group Uighur population metabolizers subjects homozygous higher than the Han group, weak metabolic type test group lower than the Han, Uygur patients consider drug clopidogrel metabolism by CYP2C19 occurs when chlorine lower probability of clopidogrel resistance, and better efficacy. 3 The results also illustrate the Han patients CYP2C19 high mutation rate, resulting in a larger proportion of poor metabolizers type, patients are more susceptible than Uygur CYP2C19 gene polymorphism. 4 Preliminary inference, gender, age, body mass index may not be the main factors CYP2C19 gene polymorphism distribution. |
