Objective:Through monitoring the level of respiratory mechanics and oxygenation of patients with acute respiratory distress syndrome,to compare the effect of endotracheal suctioning on respiratory mechanics and oxygenation of patients with acute respiratory distress syndrome undergoing different combinations of concentration and time on inhaled oxygen.To observe the improve of respiratory mechanics, finding suitable combinations of concentration and time on inhaled oxygen.Provide a theoretical basis for ARDS patients with mechanical ventilation select appropriate oxygen flow.To provide reference for endotracheal suctioning for clinical mechanically ventilated patients with ARDS.Method:A total of 30 patients with ARDS requiring mechanical ventilated were included.Application of self-control method of different concentrations and time for inhaled oxygen were conducted before and after endotracheal suction.Six combinations of concentration and time were randomized to each patient:(1)Method A,oxygen concentration(Fi O2) was 100%,oxygen time(t) for 2min;(2)Method B,Fi O2 was 100%,t for 5min;(3)Method C,Fi O2 was 60%, t for 2min;(4)Method D,Fi O2 was 60%,t for 5min;(5)Method E,Fi O2 was 40%, t for 2min;(6)Method F,Fi O2 was 40%,t for 5min.We compared Ppeak,Pplat,Pmean,Cst,Lung recruited volume,Sp O2,Pa O2,Sa O2,Pa O2/Fi O2 before and after the application of each combinations.Repeated measurement data using SPSS17.0 statistical software for statistical analysis.All measurement data are presented as mean ± standard deviation( x? S) under different concentrations and time for inhaled oxygen compared using repeated measures MANOVA.Results:1 The effect on respiratory mechanics of patients with ARDS:Parameter difference of respiratory mechanics was with statistical significance before and after endotracheal suction(P<0.05);Ppeak,Pplat and Pmean were higher than before.Lung recruited volume and Cst was lower than before;In the same time Ppeak,Pplat,Pmean,Lung recruited volume and Cst differences were not statistically significant among six groups after endotracheal suction(P>0.05).2 The effect on oxygenation of patients with ARDS:Parameter difference of Sp02,Pa02,Sa O2 and Pa O2/Fi O2 were with statistical significance before and after endotracheal suction(P<0.05);In the same time Sp02,Pa O2,Sa O2, Pa O2/Fi O2 differences were statistically significant among six groups after endotracheal suction(P<0.05).Conclusion: Endotracheal suctioning of patients with acute respiratory distress syndrome undergoing different concentrations and time of inhaled oxygen could change respiratory mechanics and oxygenation.Method E(Fi O2 was 60%,t for 2min) could more effectively and better on respiratory of endotracheal suctioning.
