Objective:Comparison of treatment between the Surgical approaches of Lateral fissure- the insular cortex and the temporal cortex about hypertensive cerebral hemorrhage in basal ganglia, providing a basis for evidence-based medicine in surgical treatment of basal ganglia hemorrhage. Materials and methods:Randomized controlled trials(RCT) were collected by CNKI, Medical, Chinese Literature Database(CBM) and Chinese Scientific Journals Database(VIP Network), which is about the treatment of basal ganglia hemorrhage by the approaches of Lateral fissure- the insular cortex and the temporal cortex. The time Interval from the establishment of databases to December 2014. Rev Man5.1 Meta-analysis software was used to evaluate the quality of the literature by Coachrane collaboration. Results:The cases of case- control study are 19. Subjects included 1609 cases, 806 cases of patients performed lateral fissure- insular surgery and 806 cases of patients performed temporal cortex treatment approach. Meta analysis show: ①GOS: Comparison of long-term prognosis,(0R= 2.81,95%CI(2.05,3.86), P<0.0001); TR group was higher than TE group. ②ADL: Comparison of early prognosis, TR group was higher than TE group,(0R = 2.15,95% CI(1.37,3.37), P = 0.0009);③postoperative complications: TR group is less than TE group( 0R = 0.34,95% CI(0.24,0.49), P <0.0001); ④rebleeding: After the surgery, rebleeding of TR group less than TE group,(0R = 0.35,95% CI(0.19,0.66), P = 0.001).mortality: mortality of TR group decreased compeared with the TE group(0R = 0.35,95% CI(0.20,0.62), P = 0.0003); ⑤mortality: mortality of TR group decreased compeared with the TE group(0R = 0.35,95% CI(0.20,0.62), P = 0.0003). Conclusions:Existing clinical data show:(1) lateral fissure- after 6 months or less, 12 months later influence insular approach and by the temporal cortex into the road on the prognosis of patients, lateral fissure- insular approach to surgical treatment of patients with a poor prognosis the incidence is lower than the temporal cortex into the road by the postoperative patient.(2) The lateral fissure- insular approach than the temporal cortex into the road by the treatment of hypertensive basal ganglia hemorrhage incidence of postoperative complications, rebleeding occurs, the postoperative mortality declined. |
