Effect Of Penehyclidine Hyrochloride And PEEP On Respiratory Function During The Gynecologic Laparoseopic Surgery

Objective:To observe the induction of clinical anesthesia during endoscopy surgery before intravenous PCHE, by making joint PEEP out surgery in patients with disease of department of gynaecology affect the influence of respiratory function in what had happened.Methods:Selected 80 patients with gynecological disease, all in all by intravenous anesthesia to laparoscopic surgery, and 80 patients were randomly divided into four group seach group 20 cases:groupⅠ(control group) was treated with intravenous injection of 1ml saline before anesthesia induction 10 min,PEEP value was set to 0cm H2O; group Ⅱ was intravenous injection of 1mg PHCE at 10 min before intubation, PEEP value was set to0 cm H2O;group Ⅲ was 1ml saline and PEEP value was set to 5cm H2O; group Ⅳ was 1mg PHCE and PEEP value was set to 5cm H2 O.Plat、Peak、Raw、Pet CO2 and arterial blood gas were recorded before injection(T0), before carbon dioxide pneumoperitoneum right away(T1),at 30min(T2),60(T3)min after pneumoperitoneum and at 10 min after pulling the endotracheal tube(T4).Results:Plat,Peak,Raw increased significantly(P < 0.01) after pneumoperitoneum in four groups,and groupⅡ<group Ⅳ<groupⅢ(P<0.01);Pet CO2 increased significantly(P<0.01) after pneumoperitoneum,and groupⅣwas less than groupⅠ(P < 0.05).T2,T3 compared with T1 monment, Pa O2(A-a DO2) decreased(increased) significantly in group Ⅰ,and tended to decrease(increase) in group Ⅱ,while to increase(decrease) in group Ⅲ,Ⅳ.Pa O2 was more in group Ⅳthan other groups at T2-4,while Pa CO2,A-a DO2(Alreolar-arterial differences foro) was lower.Conclusions:Intravenous injection of penehyclidine hyrochloride before intubation and intraoperative management with PEEP can improve The effect of pneumoperitoneum on respiratory function during gynecology laparoseopic surgery, and the overall effect is better than that of the two separate effects.
