Purpose: To investigate the value of modified sphenoidal electrodes in temporal lobe epilepsy EEG diagnosis. The interictal EEG of sphenoidal electrodes and surface of sphenoidal electrodes perform at the same time, the characteristics and differences was observed between the study of EEG in order to study different electrode in diagnosis of temporal lobe epilepsy rate.Methods:(1) research object: 32 confirmed cases of temporal lobe epilepsy patients two Affiliated Hospital of Nanchang University during the period from neural Department of internal medicine of out-patient and in-patient department, from2014 September to 2015 March, all patients underwent electroencephalography in partial sleep deprivation conditions;(2) the improved conventional sphenoidal electrode electroencephalogram + the surface of sphenoidal electrodes and sphenoidal electrodes synchronous monitoring was used for each patient. The Japanese photoelectric type EEG as EEG monitor, crocodile wire were used in the partial sleep deprivation condition to record EEG for all patients.(3) a comparative analysis of the different electrode epileptic wave like distribution; interictal discharges records of the detection rate of different stage, the frequency of different electrode, electrode discharge ratio, the average discharge wavelength; different lead different electrode tip(spine) wave detection rate were performed.(4) by t test, statistical analysis of the data, there was no significant difference of P is greater than or equal to 0.05, there were significant differences in p<0.05.Results:(1) the discharge records: the first stage was 192 times, the second stage was 176 times; the surface of sphenoidal electrodes recorded 93.75%,sphenoidal electrodes were recorded in 96.88%(p>0.05).(2) the detection rate of epileptic wave: sphenoidal electrodes modified with other 2 electrodes in patients with epilepsy wave like detection rate, there were significant differences(p<0.05),with statistical significance. The surface of sphenoidal electrodes were detected in 16cases(50%), * T1/ * T2(sphenoidal electrodes crocodile electrode connected) weredetected in 17 cases(53.13%), the conventional electrode were detected in 14 cases(43.75%).(3) the detection rate of discharge in different stages: in the first stage of the record, the surface of sphenoidal electrode detection rate was 86%, the routine EEG electrode detection rate was 70%; in the second stage; * T1/ * T2 electrode frequency was 88%, the routine EEG electrode detection rate was 54%. In addition to the surface of sphenoidal electrodes and * T1/ * T2 were no significant difference, other electrodes were significant differences(p<0.05).(4) the maximum amplitude of discharge frequency, different electrode, T2 electrode surface, * T1/ * the maximum amplitude of discharge frequency had no significant difference(p>0.05); the surface of sphenoidal electrodes and common electrodes, * T1/ * T2 electrode and routine EEG electrodes had significant difference(p<0.05).(5) the amplitude of surface discharge: Sphenoid electrode was(92.83 + 0.25) V and routine EEG electrode was(92.72 + 2.52) V, the difference of t=0.246, p>0.05, the difference was not statistically significant; the second stage of * T1/ * T2 electrode was(87.38 + 0.28) V, the average discharge amplitude of the routine EEG electrode was(87.67 + 1.02) V, the difference of t=1.551, p>0.05, the difference was not statistically significant.(6) the tip(spine) in surface wave detection rate: only sphenoidal electrodes was observed in unilateral epileptiform waves. Patients with different spike(sharp) waves with the frequency difference, emerged among 3 ~ 690(/h). Improvement of sphenoidal electrodes spike(sharp) wave frequency, amplitude and time limit and surface, * T1/* T2 and conventional EEG electrodes has significant difference(p<0.05).Conclusion: temporal lobe epilepsy EEG of sphenoidal electrode position improved, the routine EEG of sphenoidal electrodes and the surface of sphenoidal electrodes simultaneously were detect and the EEG epileptiform waves were record. The surface of sphenoidal electrodes, * T1/ * T2 electrode, T1/T2 electrode conducted a comprehensive inspection of the brain, the recorded data can better reflect the patients, the result has an important clinical value in the diagnosis of temporal lobe epilepsy.
