Installment Porous Closed Chest Drainage Treatment Of COPD And The Clinical Research Of Refractory Pneumothorax

Objective:Porous chest closed drainage in the treatment of COPD by stages and refractory pneumothorax, observe the clinical effect of the method, complications and prognosis,staging porous chest closed drainage in the treatment of COPD and the feasibility,safety and the clinical significance of refractory pneumothorax.Methods:In December of 2005 ~ 2014 were retrospectively analyzed in jiangxi university of traditional Chinese medicine affiliated hospital thoracic surgery in April a diagnosis of COPD and clinical data of 48 cases of hospitalized patients with refractory pneumothorax, divided into single and porous groups in the two groups,puckering group of 20 people, porous group of 28 people. Puckering group of patients after onset of pneumothorax puckering chest closed drainage, porous group stage under local anesthesia for patients with 3 or 4 hole closed chest drainage. Analysis of two groups of patients with serious complications and adverse reactions, led traffic,with time, the cure rate, length of hospital stay and the recurrence rate, etc.Results:Single-hole group of major complications(6/20) patients with porous group major complications(8/28), there was no statistically significant difference(P > 0.05);Single-hole group of aseptic fever incidence of porous group of patients(2/20) below aseptic fever incidence(16/28), difference was statistically significant(P = 0.001),the incidence of single-hole group above moderate pain group(3/20) below the porous medium above the incidence of pain(23/28), the difference was statistically significant(P < 0.001); Did not wipe out the chest tube death patients into or not included in the statistics, single-hole group lead flow group than in porous flow((P <0.05); not wipe out the chest tube death patients into or not included in the statistics,the single-hole group drainage time is longer than in porous drainage time(P < 0.001);the single-hole group cure rate(15/20) below the porous patients cure rate(25/28),the difference was statistically significant(P < 0.001)); Into the death of patients or not included in the statistics, single-hole group in hospital longer than porous length of hospital stay(P < 0.05); Porous single-hole group recurrence rate after curing(7/15)higher than the group of patients cured after recurrence rate(1/24), there was no statistically significant difference(chi-square = 12.115, P < 0.001).Conclusion:Application in stages of porous chest closed drainage in the treatment of COPD and the treatment of refractory pneumothorax is a valuable way. The cure rate,drainage time, rehabilitation speed than puckering chest closed drainage in treating refractory pneumothorax have more advantages, low recurrence rate, worthy of promotion, and for COPD patients with refractory pneumothorax.
