Objectives: This study was detected in the peripheral blood and pleural effusion tumor necrosis factor-ɑ(TNF-ɑ) and interleukin-2(IL-2) content of white blood cells, thereby exploring the two cytokines for tuberculous pleural the clinical value of liquid and malignant pleural effusion diagnosis.Methods: The subjects of this study a total of 90 cases were divided into healthy control group, tuberculous pleural effusion, malignant pleural effusion, 30 cases in each experiment. This experiment by the liquid multiple protein quantitative technology, also known as the Cytometric Bead Array(CBA) technology, three groups of subjects was detected in peripheral blood and pleural effusion in the content of TNF- ɑ and IL- 2. According to the results of the experiment data describe the ROC curve graph, through the area under the graph, so as to determine the TNF- ɑ and IL- 2 for tuberculous pleural effusion and differential diagnosis of malignant pleural effusion were the best threshold, further discusses the two kinds of cytokines in pleural effusion significance in differential diagnosis.Results: TNF- ɑ and IL- 2 levels in malignant pleural effusion was obviously lower than the tuberculous pleural effusion. The above two kinds of cytokines in patients with tuberculous and malignant pleural effusion peripheral blood levels and healthy controls by comparing and found no statistical difference(P > 0.05) The above two kinds of cytokines in malignant group of patients with pleural effusion and serum content is also found that there is no statistical difference(P > 0.05); But the content of TNF- ɑ and IL-2 in tuberculous pleural effusion and malignant pleural effusion in the group after the contrast, with statistical significance(P < 0.01). We can see by the ROC curve, TNF-ɑ and IL-2 best critical diagnosis of tuberculous pleural effusion values were 141.7pg / ml, 62.19 pg / ml, the sensitivity, specificity, the correct index were 89.30%, 85.80%,0.87,68.75%, 89.37%, 0.63.Conclusion: This study we were used to detect tuberculosis group, and pleural fluid levels of TNF-ɑ malignant peripheral blood and the healthy control group and IL-2 by the experimental data analysis of these two cytokines content, we can conclude the following points:1. Tuberculous pleural effusion and malignant pleural effusion TNF-ɑ and IL-2 levels were significantly higher than the corresponding peripheral blood content, which may indicate that the secretion of TNF-ɑ and IL-2 is due to the pleural cavity the stress caused by the immune response.2. Malignant pleural effusion TNF-ɑ and IL-2 levels were lower than tuberculous pleural effusion. Where the diagnosis of tuberculous pleural effusion is the most valuable TNF-ɑ, second is IL-2. Therefore, detection in pleural fluid TNF-ɑ and IL-2 levels contribute to the differential diagnosis of pleural effusion nature.
