Significance Of Serum CA19-9, CA125, CEA, ALP Separated And Combined Detection In Diagnosis And Staging Of Gallbladder Carcinoma

Part One Sensitivity of serum CA19-9,CEA,CA125,ALP separate and combined detection in the diagnosis of Gallbladder Carcinoma Objective:In recent years,due to the PCG slowly rising trend in the disease incidence of our country,some scholars researched the overall incidence of gallbladder increasi- ng by 5.5% in 2008 compared with 2001 in Shanghai.Early diagnosis is difficult for P CG,because when found it has mostly belonged to the middle or late period,and the s- urgical resection rate is low.At present,there is no specific detection index for PCG.This article is to improve the early diagnostic rate of PCG through anaLyzing CA19-9, CA125,CEA,ALP and the diagnostic and application values of four combined inspec- tion on the PCG.Methods:Retrospectively analyzed 40 cases of patients with PCG from January 2002 to December 2012 in the Affiliated Hospital of Qinghai University’s general surgery department as malignant group,40 patients with benign gallbladder diseases who were hospitalized for the same period as the benign group,and outpatient medical 40 cases as control group at the same period,using the American Abbott I 2000 automatic che- miluminescence immune analyzer test three sets of serum CA19-9,CEA,CA125,ALP, analysis of the sensitivity of the above indicators separate and four with to the detecti- on for the diagnosis of PCG.Results:Compared with gallbladder benign lesions and the healthy check-up, levels of PCG serum CA19-9,A125,CEA,CALP raised(P < 0.05);CA19-9,CA125,CEA,ALP combined detection in the diagnosis of PCG sensitivity,positive predictive value and accuracy were higher than single detection.Conclusions:PCG serum CA19-9,CEA,CA125,ALP combined detection can improve the gallbladder carcinoma for diagnostic rate. Part Two Clinical stage of serum CA19-9,CA125,CEA,ALP separate and combined detection for Gallbladder CarcinomaObjective:PCG is the most common malignant tumor in malignant tumors of the biliary system,accounts for about two-thirds.It is lack of specific clinical features in early gallbladder carcinoma,early diagnosis rate is low,and it is easily confused with Chron- ic Cholecystitis and Cholelithiasis and gallbladder polyps and other benign lesions,be- cause the gallbladder conceals,progress rapidly,it has belonged to the middle or late period when clinical symptoms,at this time the patient is line low operation rate and lo- w 5-year survival rate after the operation.But early gallbladder surgery prognosis is good.The 5-year survival rate can reach l00%.In this article,in order to improve the early diagnostic rate of gallbladder,for clinical staging and preoperative evaluation of P- CG and the operation method choice,improve the prognosis,by focusing on analysis of PCG serological indexes CAl99,CAl25,CEA,ALP,explore serum indicators associ- ated with PCG diagnosis.Methods:Retrospectively analyzed 68 cases of patients with PCG from January 200 2 to December 2012 in the Affiliated Hospital of Qinghai University’s general surger- y department,they were all confirmed for surgery or postoperative pathologic examin- ation.We compared and analyzed serumCAl9-9,CAl25,CEA,ALP of patients with P- CG in different TNM staging.They are divided into no regional lymph node metastas- is group(Stage I+ StageⅡ+StageⅢA),regional lymph node metastasis group(StageⅢB),distant lymph node metastasis group(StageⅣ),according to whether the TNM staging with lymph node metastasis.We divided into feasible radical excision group(Stage I+Ⅱ+ⅢA) and not feasible radical excision group(Stage ⅢB+Ⅳ) according to TNM staging,according to whether it is feasible to radical. Results:There were statistically significant for serum CAl9-9,CAl25,CEA and ALP level compared with the difference between 3 groups Presence of lymph node metastasis.No regional lymph node metastasis group serum CA19-9 and CEA positive rate and ALP abnormal rate was significantly lower than that of regional lymph node met- astasis and distant metastasis group(P < 0.01),serum CA125 positive rate was signifi- cantly lower than distant metastases group(P < 0.01).Feasible radical excision group CAl9-9 and CEA level and the positive rate and serum ALP level and abnormal rate were significantly lower than not feasible radical excision group(P < 0.01).No obvious difference was found between serum CA125 levels and the positive rate.CAl9-9, CAl25,CEA,ALP positive rate of four joint detection was obviously higher than that of single detection explicit rate(P < 0.01).Conclusions:PCG serological CAl99, CAl25, CEA, ALP joint detection has certain clinical application value as an auxiliary diagnosis method and the clinical staging and preoperative evaluation of PCG,combined detection can improve the gallbladder carcnoma for diagnostic rate.
