ObjectiveTo assess the respiratory nursing interruption frequency, source, the main body role, type and ending during medication administration, analysis the nursing interruption related influencing factors, to explore nursing interruption of effective management methods, to improve clinical nursing safety of medication administration providing guidance. MethodUsing“Respiratory nursing interruption in medication administration registration form”, Fujian Medical University Union Hospital Respiratory medicine process nursing interruption was investigated from may to september in 2014, Mainly includes three key links: nurses treating physician order(physician order processing group), nurses dispensing medicines(medicine dispensing group), nurses administering medicines(medication administering group).All of study information or data was collected by direct observations and inquiries; Main observation content source, the main body role classification, type and ending of interruption during medication administration; Builds up the data base and manages all the data with the software Excel, use spss22.0 software for statistical analysis. ResultsDuring medication administration, a total of 846 nursing interruption was observed in 108 hours,7.8 per hour on average.10.8 per hour in physician order processing group,4.2 per hour in medicine dispensing group,8.5 per hour in medication administering group.Major source of nursing Interruption: environment(26.7%),family(25.5%),doctors(15.7%);Subject role classification of nursing interruption:expected to accept type(36.7%)in physician order processing group, attention dispersible type(52.7%)in medicine dispensing group, expected to accept type(40.5%)in medication administering group; Nursing interruption types: interruptive type(42.4%),distracting type(26.1%),damaging type(22.8%),conflicting type(8.7%);Ending of nursing interruption: negative type(89.1%),positive type(10.9%),with the highest(97.4%)in medication administering group, mainly increase nurses’ working load(61.1%)in negative type, followed by the suspension of the current task(29.7%);In the study 12 cases of medication potential errors because of nursing interruption was observed.In addition to the negative interruption distribution difference has no statistical significance(p > 0.05).Source, the main body role classification, type and ending of interruption in three key links distribution difference have statistically significant(p < 0.05). Conclusion 1.The frequent nursing interruptions occurred in respiratory nursing medication administration. 2.Sources of nursing interruption in the respiratory medication administration was complex, physician order processing group mainly comes from the environment and doctors, medicine dispensing group mainly from family members and the environment, medication administering group mainly from patients or their families and nurses’ colleagues. 3. Main body role classification of interruption mainly were type of expected to accept and initiated. 4.Type of interruption mainly was interruptive type, followed by distractions. 5.Negative outcome in respiratory nursing interruption was common, and may potentially lead to medication errors.Therefore, To ensure medication administration safety, it was very significant for respiratory nurses to avoid or reduce negative nursing interruption effectively. Measures shall be implemented including improve the consciousness of nursing interruption risk assessment, strengthen the psychological quality of resistance to interrupt, use warning mark and set full-time receptionist, etc.
