Objective:Comparison and analysis of laparoscopy and laparotomy under D2 radical gastrectomy for gastric cancer patients with peritoneal exfoliated cells, and provide clinical evidence and reference for the selection of operation in patients with gastric cancer in clinical work in the future and the evaluation of prognosis, and explore the clinical significance of detection of gastric cancer peritoneal exfoliated cells. Methods:Collected 30 cases by laparoscopic D2 radical gastrectomy for gastric cancer patients before operation, in peritoneal lavage fluid, 32 cases with open downward D2 radical gastrectomy for gastric cancer patients before operation, in peritoneal lavage fluid, Because flow cytometry(flow cytometry, FCM) One feature is the use of DNA content and ploidy judgment of benign and malignant cells, we take advantage of this feature to be used for peritoneal lavage cells exfoliated tumor detection, and the measured results show that DI≠1 judged as positive.according to the number of specimens collected divide in to groups and correspongding pathological examination. Results:( 1) The lavage fluid cytology positive rate in preoperative la-paroscopic peritoneal was 32.50%(13/40). In The open group pr-eoperative the positive rate was31.42%(11/35), there is no significant difference with the laparoscopic group(P>0.05).( 2) The lavage fluid cytology positive rate after operative laparoscopic peritoneal was 37.50%(15/40),and there is no difference with the rate in preoperative. In the open group operative thepositive rate was 37.14%(13/35),have no significant difference with the laparoscopic group(P>0.05).Conclusion:( 1) Laparoscopy gastrectomy and open gastrectomy for the gastric cancer under D2 radical gastrectomy for gastric cancer patients had no significant differencein the positive rate of detection of exfoliated cells, the laparoscopic compared with open operation is minimally invasive advantages in clinical work, more suitable for radical resection of gastric cancer.
