Objective:Observe the hemocoagulase in extracorporeal circulation in heart surgery, postoperative bleeding effect, and tranexamic acid hemostatic efficacy.Method : Choose 60 cases of child with congential heart diseaseincluding ASD(34),VSD(20),ASD with VSD(6),except for complex congenital heart disease(TOF).Aged for 2 to 14 years old(average 6.12 years old),31 males and 29 females,randomly divided into A,B,C three groups,each group includes 20 cases,of which group A for blood clotting enzyme(30 minutes) to give the hemocoagulase group;group B for blood clotting enzyme(30 minutes) to give the tranexamic acid.The control group is the C group.Set the Ⅰ,Ⅱ,Ⅲ three sampling points: Ⅰ point for preoperative 3 days of hemoglobin(Hb), hematocrit(Hct),activated partial clotting enzyme time(APTT),prothrombin time(PT),D-dimer(DD),fibrinogen degradation products(FDP) data;Ⅱ,Ⅲvpoint respectively and postoperative 1 hour and 3 hour of sampling points after surgery,respectively monitoring post-operative Hb, Hct, APTT, PT, DD, FDP, chest fluid volume(ml).The above groups in every delivery in accordance with the illustrations of the hemocoagulase and tranexamic acid dosing,and record the data.Result:All patients participated in the experiment were all successfully completed, and there were no postoperative bleeding, arrhythmia, heart failure and other serious complicationg and even death. Because of A,B,C three groups are small sample measurement data,measurement data with mean ± standard deviation(?x±s) said,group between group’s comparison of measurement data in the group using single factor analysis of variance(one-way ANOVA).Comparison between any two grops take SNK-q test.It is concluded that the detection index respectively measured F and q.Drew the P value and conclusion.(1) After statistics analysis:the experimental group of preoperative Hb,Hct,PT,APTT,DD,FDP measured values F, it is concluded that P>0.05,as a result, the index of preoperative is not significant,therefore conform the design.(2) Two group of the experimental group compared with blank group, the Hb,Hct,PT,APTT,DD,FDP,chest fluid volume measured values F,so the P<0.05,the comparison between groups has statistically significant.(3) Comparing the twe experimental groups, the hemocoagulase group compared with the tranexamic acid group,F value of each index all>1,it is concluded that P<0.05,the comparison of the experimental group was statistically significant. Inspection level take α= 0.05。Conclusion:(1) Hemocoagulase and Tranexamic acid can be used for stop blood in extracorporeal circulation in heart surgery;(2) Hemocoagulase have a good hemostasis effect than hemostatic drug Tranexamic acid.
