Background: Renal cell carcinoma(RCC) in the renal tubular epithelium of urinary malignant tumors, high malignancy, high mortality, more and more attention, and is closely related to many factors, most of think environment, obesity, industrial material(CD), high blood sugar, high blood lipids, the use of related drugs [2], part of Chinese herbal medicine and related chronic nephritis nephropathy have certain relation at present. From the angle of the occurrence and development of tumor, the program is very complex, there are generally many factorsfrom the biological point of view, to study the cause of the molecular biological mechanism of the occurrence and development of renal cell carcinoma, and to evaluate the prognosis of renal cell carcinoma is present in renal cell carcinoma of basic and clinical research focus. The researches about the relationship between renal cell carcinoma and cytokines is a hot research field of molecular biology of renal cell carcinoma. Objective:Analysis of the expression of P19 in renal cell carcinoma, and to exploring the relation between P19 and the development and the clinical pathological feature of renal cell carcinoma, clinical analysis of Gem+Sor+Bev combined with chemotherapy as clinical effects of advanced renal cell carcinoma failed previous II and IIF treatment of three line regimen in the treatment of postoperative adverse reaction and the corresponding. Methods : To detect the expression of P19 protein by IHC method in 62 cases of renal cellcarcinoma and 15 cases of non tumor kidney tissues, combined with the data of statistical analysis of clinical pathology; analysis of the P19 gene and protein in 30 cases of renal cell carcinoma and 10 cases of non tumor kidney tissues express RT-PCR and the application of Western-blot method during the period of 2013 October to 2014 October through II and IIF treatment failure or poor effect and patients intolerant of advanced colorectal carcinoma of kidney in patients with a total of 48 cases, of which 30 application of chemotherapy of Gem+Sor+Bev regimen(gemcitabine):Gemcitabine 1.0/m2, d1,8,15, Bevacizumab(bevacizumab) 0.2/m2,d1,14, Sorafenib(Sola Feeney) 0.2, bid, Po, d1-21, for 28 days a treatment cycle, evaluation of curative effect after three weeks, every 2 cycles assessment andsummary of its adverse reactions, such as chemotherapy tolerance, the condition improved or no progress of deterioration in general, cycle to six cycles is appropriate. Results : P19 has obvious difference in expression in two tissues, the expression of P19 gene and protein in renal cell carcinoma tissues was significantly lower than that of the matched normal non tumor tissues(P < 0.05). The curative effect by six weeks later and return the results of observation and study, long term, T group and N group, the median progression free survival(mPFS) were 12.5 and 3.5 months, and median overall survival(mOS) were 10.1and 3.4 months, the two groups had significant differences(P<0.05).Conclusion:The expression level of P19 gene and protein in renal cell carcinoma, which has great value in judging the occurrence and metastasis, can be used as a clinical course of renal cell carcinoma potential index, provide the basis for the treatment and prognosisevaluation, Gem+Sor+Bev three line treatment of failure of II and IIF for treatment of advanced renal cell carcinoma overall clinical efficacy, the majority of patients have bad reaction which can be tolerated, the median survival period was prolonged, promote the use of the scheme can be clinically.
