Objective This study was to explore the relationship between lymph node metastasis and pathological factors in superficial esophageal cancer.Method Refer to all surgically resection specimens of pathological data from1999 to 2012 in anyang city of henan province tumor hospital, select one of the superficial esophageal cancer and eventually into the group of the number of cases 2211 cases, all cases of preoperative without radiation and chemotherapy treatment, namely using surgical resection for the first time. Carefully check all the clinical and pathological data, and all sections were reviewed by two associate pathologists or chief pathologists. According to the unified standard classification, we analyzed each pathological factors of lymph node metastasis, which was divided into two groups: no lymph node metastasis and lymph node metastasis. Supervision immunohistochemistry method was used to detect the expression of VEGF-C protein、KAI1/CD82 protein and Ki-67 antigen in 40 cases of no lymph node metastasis group and 32 cases of lymph node metastasis group,which were randomly selected. In accordance with the general standard to judge the negative and positive results, this study was to explore the expression of VEGF-C protein、KAI1/CD82 protein and Ki-67 antigen,and to analysis their correlations with lymph node metastasis and clinical pathological parameters.Results From 2211 cases, tumor infiltration depth, tumor type, tumor histological grade and the presence of tumor emboli were closely associated with lymph metastasis(positive correlation), namely, patients were prone to lymph node metastasis, whose tumor infiltrate into the submucosa, general type was plaque or erosion, histological grade was higher and tumor emboli, whereas other pathological factors showed no relation with lymph node metastasis, for example sex, age, tumor location, the maximum diameter of tumor, tumor histological type. The lower expression of KAI1/CD82 protein, the higher expression of VEGF-C protein and the higher coloring index of Ki-67 antigen(more than 20%)were easy to transfer.Conclusion For those tumor infiltrating into the submucosa, general type was plaque or erosion, higher histological grade and tumor emboli, at the same time combined with immunohistochemical, the lower expression of KAI1/CD82 protein, the higher expression of VEGF-C protein and the higher coloring index of Ki-67 antigen(more than 20%)hint that lymph node may be metastatic. All kinds of examination should be actively improved, including endoscopic ultrasonography examination and laser confocal endoscopy technology, CT and MRI and PET-CT examination, to confirm the presence of lymph node metastasis.
