Objectives Considering diabetic bladder emptying disorders clinical and urinary tract obstruction factors(male prostate hyperplasia, female bladder neck hypertrophy), by applying the color ultrasound technology measure bladder residual urine volume of the object of study, To analysis diabetes on the effect of bladder emptying functio n; From imaging and clinical indicators to analysis the influence factors of diabetic bladder emptying disorders and its clinical complications, aimed at ultrasound screening diabetic bladder emptying dysfunction of high-risk groups, provide valuable imaging information for clinic.Methods From January 2014 to October 2014 in tangshan workers’ hospital endocrinology in hospital, and met inclusion and exclusion criteria, 136 patients with diabetes as diabetes group.Imaging observation indexes: bladder resid ual urine volume, prostate volume, the width proportion between internal and external gland, Intravesical prostatic protrusion; Female bladder neck width and thickness. C linical observation indexes: age, duration of diabetes, body mass index, blood pressure, blood sugar, glycosylated hemoglobin, blood lipid, blood urea nitrogen, creatinine, combine the peripheral neuropathy. To observe the clinical complications of urinary system indicators: the degree of separation of renal collecting system, roughness of bladder mucosa trabecular, small net formation and urine bacteria test results. Select period of other departments in the hospital, met inclusion exclusion standard 88 nondiabetic patients as control group.Is diabetic and non-diabetic group bladder residua l urine volume if there is a difference, According to the hyperplasia of prostate or not, more men with diabetes and male control bladder residual urine volume difference; According to whether the bladder neck hypertrophy, compare women patients with diabetes and control group in bladder residual urine volume difference; With bladder residual urine volume is greater than 50 ml for cutoff point was divided into 2 groups, analysis of imaging, clinical diabetic patients and clinical complications of urinary system.Input the collected data into Excel software, set up the database, using SPSS 17.0 statistical software for statistical analysis, test of goodness of fit with K-S analysis of measurement data of normality, data conforms to normal distribution using independent sample t-test to compare between two groups; Non-normal distribution data comparison between the Mann- Whitney U test.; C hi-square analysis of measurement data application, multi- factor analysis using multiple stepwise linear regression.All the inspection are bilateral, with P < 0.05 for the difference was statistically significant.Results 1 Diabetes group of bladder residual urine volume is greater than the control group;After considering urinary tract obstruction factors, patients with diabetes Bladder residual urine volume is still greater than the control group.2 Compared with the residual urine group, men with diabetes group in the course of residual urine, volume of prostate and bladder prostate direction prominence, blood urea nitrogen, creatinine, merging were higher;Men with diabetes bladder residual urine volume and Peripheral neuropathy was positively related to relationship, χ2=6.203, the difference was statistically significant(P<0.05), while the rest of the difference between the two groups. Multiple stepwise linear regression showed that have significant effects on male diabetic bladder emptying dysfunction of variables for the duration of diabetes, prostate bladder direction prominence and creatinine.3 Compared with the residual urine group, women with diabetes group in the course of residual urine, glycosylated hemoglobin, blood urea nitrogen, creatinine, bladder neck thickness is higher;Women with diabetes bladder residual urine volume and Peripheral neuropathy was positively related to relationship, χ2=6.203, the difference was statistically significant(P<0.05), while the rest of the difference between the two groups. Multiple stepwise linear regression showed that the female diabetic bladder emptying dysfunction has a significa nt impact on variable for diabetes duration, the thickness of the bladder neck and blood urea nitrogen.4 The residual urine bladder group of diabetic’ separation of renal collecting system, bladder trabecular, small net forming, the incidence of urinary tr act infection, and enlargement of bladder mucosa were higher than in the residual urine group.Conclusions 1 After considering urinary tract obstruction factors, diabetes is still affect bladder emptying function, residual urine volume increased. 2 The lo ng course of the disease in patients with diabetes, and hyperplasia of prostate(male) or bladder neck hypertrophy(women) are likely to cause bladder emptying dysfunction. 3 Many people with diabetes residual urine volume, are more likely to cause urinary tract infections, change of shape and structure of the bladder, and separation of renal collecting system, etc. A series of clinical complications.
