Objectives To investigate e NOS gene polymorphism and occupational noise environment combined impact on the essential hypertension in steel workers, and reveal the cause of essential hypertension, in order to provide a scientific evidence for prevent and control high blood pressure,and alleviate individuals and businesses burden.Methods According to the inclusion and exclusion criteria, 76 cases and 376 healthy worker ware selected from 2165 workers who ware take annual examination in Group Occupational Disease Prevention Hospital during Feb.to August 2014. Hypertension diagnosis of all subjects are in line with 2010, "Hypertension Prevention Guide." The questionnaire collected demographic data related to clinical data, occupational history,personal history, past medical history,etc., determination of total cholesterol(TC),triglyceride(TG), fasting blood glucose(FBG), low-density lipoprotein(LDL), high density lipoprotein(HDL). Meanwhile, on-site measurements and calculated 40h(LAeq)according to the distribution of the plant and trades. By polymerase chain reaction-restriction fragment length polymorphism analysis(PCR-RFLP) or polymerase chain reaction(PCR) for all study subjects e NOS gene rs1799983, rs1800780 and 27 bp VNTR gene polymorphism analysis.Multivariate Logistic regression analysis genes and environment factors effect on hypertension.Results 1 The genotype of case group and control group were consistent with HardyWeinberg equilibrium(P>0.05). 2 For rs1799983 cite, the frequencies of GT genotype respectively were 25.5%, 20.5%, TT genotype was 7.7%, 4.8%, and T allele frequencies were 20.5%,15.0% in case and control groups, there was no significant difference(P>0.05). 3 For rs1800780 cite, the frequencies of AG genotypes were 54.0%,50.3% respectively, GG genotype was 32.4%, 19.9%, and G allele frequencies were59.4%,45.1% in case and control groups there was statistically significant difference(P<0.05). 4 For 27 bp VNTR cite, the frequencies of ab genotypes were 33.0%,26.6% respectively, aa genotype was 4.5%, 0.8%, and a allele frequencies were21.0%,14.0% in case and control groups, there was statistically significant difference(P<0.05). 5 Multivariate Logistic regression take whether hypertension as thedependent variable, respectively rs1799983, rsl800780 and 27 bp VNTR cites of e NOS gene as independent variable, after adjusted for smoking, alcohol consumption, physical exercise, showed that 27 bp VNTRG,BMI and 40h(LAep) level was independent of hypertension risk factors, showed that 27 bp VNTR genes,BMI and noise exposure40h(LAep) respectively increased risk of hypertension 3.44(95%CI:1.90-6.23) times,1.16(95%CI:1.07-1.25) times, 2.15(95%CI:1.23-3.75) times. 6 Multivariate Logistic regression analyze showed that there are positive interaction between rs1800780 and27 bp VNTR, ORint=6.24(95%CI:3.11-9.38). There are significant positive interaction noise exposure and 27 bp VNTR, ORint=6.19(95%CI:4.22~9.33).Conclusions 1 The 27 bp VNTR polymorphismo, BMI and 40 h LAep may be risk factors for hypertension in the occupational groups. 2 There are positive interactive between rs1800780 and 27 bp VNTR gene, nosie exposure and 27 bp VNTR gene, that increase the risk of essential hypertension in the occupational groups.
