| Objective To observe the clinical effects and security of Qing Shen Fang integrated with foreign medicine into the therapy of active lupus nephritis,so as to explore the mechanism of the treatment of Qing Shen Fang and explore the preponderance on the treatment of LN,and investigate the level of serum HMGB1 and IL-17 in patients with LN and its significance through the research,and providing a logical proofs to the clinical practice and generalize use.Methods The 60 patients diagnosed with clinically active lupus nephritis were all randomly divided into two groups,the qing shen fang group and mached group,both had 30 der.ALL were donor an intravenous drip of cyclophosphamide and oral Bonny loose piece,the treatment group was given an extra Qing Shen Fang,taking 2 times daily, three months for one period of treatment.The other treatments of the two groups were basically same.By surveying the curative therapeutic effects,SLEDAI,quantitative test of 24 h urinary prot ein,u RBC,ESR, C3,C4,Ig G,ALB and Scr,Hb,ANA,Anti ds-DNA antibody, HMGB1,IL-17, and statistical analysis to summarized the clinical therapeutic effect. Observe the safety index of three conventional, hepatic-renal function.To Compare the rate of adverse reaction.Result ○1 Comprehensive of curative effect analysis: in the treatment group,5 sufferer were clinical complete remission,12 were markedly valid and 11 were valid,the overall response rate was 93.3%.In the control group,1 sufferer was clinical complete remission,8 were markedly valid and 16 were valid,the overall response rate was 83.3%,P<0.05,the treatment group showed meliorbetter.○2 Chinese medicine symptom integral and the disease activity comparison:two groups of patients with symptoms after treatment were improvement,total symptom integral and SLEDAI scores compared with the previous were fall,P<0.05.And symptoms improve the SLEDAI score and total integral treatment group were better than control group(P<0.05).○3 Main laboratory indexes after treatment in both groups(24 hours urinary protein, erythrocyte sedimentation rate, serum albumin, C3,C4 and Ig G) was improved markedly,P<0.05,and the treatment group is better than the control group, P<0.05.But in antinuclear antibodies, ds-DNA antibodies, there was no statistically significant difference between the two groups(P>0.05).○4 The serum levels of HMGB1 and IL-17 though in two groups were all improved after the treatment,and it indicated the differences were statistically significant(P<0.05). ○5 Safety observation:In clinical observation,adverse reactions cases in treatment group were lower than the control group, The rates of the adverse reaction of the two groups were 6.7% and 26.7%,P<0.05.There was no termination of treatment in the two groups,all can continue accept the treatment after symptomatic treatment,and the patients could tolerate.We also didn’t observe adverse reactions associated with Qing Shen fang.Conclusion The effect of Qing Shen Fang combined With conventional western medicine therapy had more significance compared with western medicine treatment.It can improve the clinical curative effect, alleviating patients clinical manifestation,improve laboratory index and reduce disease activity. At the same time have a unique gentle security features, natural medicines and less adverse reactions and the advantages of light,is worth clinical promotion.Its mechanism may involve the lower expression of inflammatory cytokines HMGB1 and IL-17, regulation immune responses, reducing tissue damage,in order to improve the treatment of therapeutic effect. |
