Peripheral Blood Leukocytes Are Associated With Prognosis After Intracerebral Hemorrhage

Purpose : To investigate the levels of peripheral blood leukocyte changes after intracerebral hemorrhage(ICH) and to explore the relationship between level changes of peripheral blood leukocytes and prognosis after ICH.Methods: 120 cases of ICH patients were collected from April 2012 to December 2014 in the Department of Neurology at the First Affiliated Hospital of Henan University. There are 4 groups of those patients: 1. volume of blood in the brain < 30 ml; 2. volume of blood > 30 ml; 3. blood enter into ventricle(intraventricular hemorrhage); 4. expanded hematoma; and the normal control group. We examined the peripheral white blood cells from the venous blood when the patients entered into the hospital; afterwards the peripheral white blood cells were reexamined at days 3, 7, 14 in all ICH patients. The ICH patients with the different GCS scores were divided into light, medium and heavy groups and were used to compare the change level of white blood cells. The ICH patients after recovery were divided into good, bad(severe disability and plants live), and death three groups to investigate the change levels of white blood cells.Results: The peripheral blood leukocyte and neutrophils with ICH patients were significant higher(P < 0.05) than that of normal control group; lymphocytes were significant lower than the normal control group(P < 0.05). The neutrophils in intraventricular hemorrhage patients were higher than the hematoma < 30 ml patients with an obvious significant difference(P < 0.01). The white blood cells in ICH patients were increased when the ICH patients entered into the hospital; the changes of white blood cell were closely related to the patients’ illness severity at 3, 7, 14 days after ICH. There is a significance difference(P < 0.05) in medium type of ICH patients compared with light type patients. There is a remarkable difference(P < 0.05) in heavy type of ICH patients compared with light type patients. There is a significance difference(P < 0.05) in medium type of ICH patients compared with heavy type patients. The leukocytes were significant lower(P < 0.05) in the good recovery group compare to the poor recovery and death group. Compared to the different parts of the brain hemorrhage and white blood cell count, we found that the white blood cells were significant(P < 0.05) increased in the large volume of hemorrhage in basal ganglia, also this increased white blood cells were related to the blood entered into the ventricle. Lobe hemorrhage had a less leukocyte increase compared to other parts of the hemorrhage(P < 0.05),Conclusions:1. The peripheral white blood cells in the early period of ICH patients were increased, which were closely associated with ICH severe degree. These data indicated that the peripheral white blood cells are one of the indicators of prognosis of ICH patients. 2. ICH patients have higher white blood cells compare to normal control group. The increased white blood cells may be related to the location, volume, and time after ICH.
